摘　　要：目的:分析首发抑郁症患者静息态功能磁共振成像(rs-fMRI)特征,探讨rs-fMRI与患者睡眠质量及记忆水平的相关性。方法:选取在医院就诊的21例首发抑郁症患者作为观察组,同期招募的25名健康志愿者作为健康对照组,应用核磁共振成像(MRI)进行静息态血氧水平依赖(BOLD)扫描,采集局部一致性(ReHo)和比率低频振幅(fALFF)值,观察评价其睡眠质量评分和记忆水平的相关性。结果:观察组在左侧额下回、右侧中央前回、左侧颞中回以及左侧中央前回与额中回ReHo值明显高于对照组(t=8.952,t=6.262,t=8.244,t=10.025;P<0.01),左侧枕中回、双侧额直回、右侧颞上回以及右侧颞中回ReHo值明显低于对照组(t=7.106,t=8.633,t=6.242,t=8.915;P<0.01),左额上回、左侧额下回、右侧海马旁回、左侧颞中回、左侧前扣带回、右侧中央前回、左侧中央旁小叶fALFF值高于对照组,在双侧尾状核fALFF值低于对照组(t=14.162,t=7.284,t=6.155,t=8.946,t=7.162,t=5.926,t=9.166,t=7.085;P<0.01)。左侧额下回RoHo值与匹兹堡睡眠质量指数量表(PSQI)评分呈正相关(r=0.50,P<0.01),观察组中左侧额上回fALFF值与PSQI呈正相关(r=0.41,P<0.05),左侧额下回fALFF值与PSQI呈正相关(r=0.27,P<0.05),右侧海马旁回fALFF值与PSQI呈正相关(r=0.46,P<0.05)。左侧额下回RoHo值与短时记忆呈负相关(r=-0.17,P<0.05),左侧前扣带回fALFF值与短时记忆呈负相关(r=-0.2,P<0.05),双侧尾状核fALFF值与短时记忆呈正相关(r=0.59,P<0.01)。结论:首发抑郁症患者脑部存在广泛的ReHo、fALFF异常,且部分脑部ReHo、fALFF异常值与睡眠质量和记忆水平呈现相关性。Objective:To analyze the features of resting-state functional magnetic resonance imaging(rs-fMRI)in patients with first-episode depression,and to further explore the correlation of rs-fMRI and sleep quality,and the correlation of that and memory level.Methods:A total of 21 patients with first-episode depression were selected as the observation group,and 25 healthy volunteers were selected as the control group at the same time.MRI was applied to conduct blood oxygen level dependent(BOLD)scan of resting-state,and gather the values of regional homogeneity(ReHo)and fractional amplitude of low frequency fluctuation(fALFF).And then,the correlations of that and the score of sleep quality,and the level of memory were observed and evaluated.Results:Compared with control group,the ReHo values of the left side of gyrus frontalis inferior,the right side of anterior central gyrus,the left side of gyrus temporalis medius and the left side of anterior central gyrus/middle frontal gyrus of observation group were significantly increased(t=8.95,t=6.26,t=8.24,t=10.02,P<0.01),while the ReHo values of the left side of middle occipital gyrus,bilateral straight frontal gyrus,the right side of superior temporal gyrus and the right side of middle temporal gyrus of observation group decreased significantly(t=7.10,t=8.63,t=6.24,t=8.91,P<0.01).The fALFF values of the left side of superior frontal gyrus,the left side of inferior frontal gyrus,the right side of parahippocampal gyrus,the left side of middle temporal gyrus,the left side of anterior cingulate gyrus,the right side of anterior central gyrus and the left side of paracentral lobule of observation group significantly increased,and that of bilateral caudate nucleus significantly decreased(t=14.162,t=7.284,t=6.155,t=8.946,t=7.162,t=5.926,t=9.166,t=7.085,P<0.01),respectively.In addition,the RoHo value of the left side of inferior frontal gyrus was positively correlated with the score of Pittsburgh Sleep Quality Index(PSQI)(r=0.50,P<0.01).The fALFF value of the left side of superior fron
