化痰通络颗粒与rt-PA静脉溶栓联用对急性缺血性脑卒中患者NIHSS评分、运动功能评分的影响

摘　　要：目的 探究急性缺血性脑卒中患者采用化痰通络颗粒与阿替普酶(rt-PA)静脉溶栓联合治疗的临床效果。方法 依据随机数字表法对2020年5月-2021年5月山东省青岛市黄岛区中医医院收治的80例急性缺血性脑卒中患者分组(每组40例)进行探究,对照组采用rt-PA静脉溶栓,观察组在对照组的治疗基础上加用化痰通络颗粒,对比两组美国国立卫生研究院卒中量表(NIHSS)评分、血管神经修复因子、Fugl-Meyer量表(FMA)评分。结果 治疗前,两组NIHSS评分、血管神经修复因子、FMA评分差异无统计学意义(P> 0.05);治疗后,观察组NIHSS评分低于对照组(P<0.05);观察组血管内皮生长因子(VEGF)、脑源性神经营养因子(BDNF)指标高于对照组(P<0.05),且神经元特异性烯醇化酶(NSE)、血管细胞黏附分子(VCAM-1)指标低于对照组(P<0.05);观察组FMA评分高于对照组(P<0.05)。结论 化痰通络颗粒联合rt-PA静脉溶栓可促进血管修复,改善神经功能,临床应用效果明显。Objective To explore the clinical effect of Huatan Tongluo Granules combined with recombinant tissue plasminogen activator(rt-PA) in patients with acute ischemic stroke. Methods According to the random number table method, 80 patients with acute ischemic stroke treated in Huangdao District Hospital of Traditional Chinese Medicine, Qingdao City, Shandong Province from May 2020 to May 2021 were divided into a control group and an observation group, 40 cases in each group. The control group was treated with rt-PA, while the observation group was given Huatan Tongluo Granules on the basis of the control group. Then the National Institutes of Health Stroke Scale(NIHSS) score, vascular nerve repair factors, and Fugl-Meyer assessment(FMA) score between the two groups were compared. Results Before treatment, there was no difference in the NIHSS score, vascular nerve repair factors and FMA score between the two groups(P>0.05). After treatment, the NIHSS score and neuron-specific enolase(NSE) and vascular cell adhesion molecule(VCAM-1) indexes of the observation group was lower than those of the control group(all P<0.05), while the vascular endothelial growth factor(VEGF) and brain-derived neurotrophic factor(BDNF) were higher than those of the control group(P<0.05);the observation group had higher FMA score than the control group(P<0.05). Conclusion Huatan Tongluo Granules combined with rt-PA could promote vascular repair and improve nerve function, with a significant effect in clinical application.
