巩膜隧道切口小梁切除术对急性闭角型青光眼的临床价值研究

摘　　要：目的分析巩膜隧道切口小梁切除术对急性闭角型青光眼治疗方面的临床价值。方法本次研究选择2018年1月至2019年12月期间在本院接受治疗的54例急性闭角型青光眼患者,并且按照入院单双号时间将患者划分为观察组和对照组,每组27例。对照组选择常规小梁切除术,观察组则采用巩膜隧道切口小梁切除术,比较两组患者手术前后的眼压状态和视力恢复情况,分析巩膜隧道切口小梁切除术在治疗方面起到的稳定效果。值为(28.52±0.72)mmHg,对照组为(28.56±0.67)mmHg,差异无统计学意义(t=0.159,P=23.358);(2)手术后,观察组眼压值为(14.04±0.46)mmHg,对照组眼压值为(18.72±0.51)mmHg,差异有统计学意义(t=3.881,P<0.05)。(2)视力(1)手术后视力>0.4,观察组12例,对照组6例,差异有统计学意义(χ^(2)=5.977,P<0.05);(2)手术后视力为0.1-0.4,观察组15例,对照组17例,差异无统计学意义(χ^(2)=1.014,P>0.05);(3)手术后视力为<0.1,观察组0例,对照组4例,差异有统计学意义(χ^(2)=5.801,P<0.05)。结论在急性闭角型青光眼患者的治疗过程当中,采用巩膜隧道切口小梁切除术既可以有效稳定的降低眼压,提高患者的视力,且疗效显著值得推广应用。Objetive To analyze the clinical value of trabeculectomy through scleral tunnel incision in the treatment of acute angle-closure glaucoma. Methods In this study, 54 patients with acute angle-closure glaucoma who were treated in our hospital from January 2018 to December 2019 were selected, and were divided into observation group and control group according to the admission time, with 27 cases in each group. The control group was given routine trabeculectomy, while the observation group was given trabeculectomy through scleral tunnel incision. The intraocular pressure status and vision recovery before and after operation were compared between the two groups, and the stable effect of trabeculectomy through scleral tunnel incision in treatment was analyzed.Results(1)Intraocular pressure(1)Before surgery, the IOP in the observation group was(28.52±0.72)mmHg, while that in the control group was(28.56±0.67) mmHg,the difference was not statistically significant(t =0.159, P = 23.358).(2) After surgery,the IOP of the observation group was(14.04±0.46) mmHg, while that of the control group was(18.72±0.51) mmHg,the difference was statistically significant(t =3.881, P < 0.05).(2)Visual Acuity(1)Visual acuity > 0.4 after surgery, was observed in 12 patients in the observation group and 6 in the control group, the difference was statistically significant(χ^(2)=5.977, P<0.05).(2)Visual acuity of 0.1-0.4 after surgery, there were 15 cases in the observation group, there were 17 cases in the control group,the difference was no statistically significant(χ^(2)=1.014, P> 0.05).(3) Visual acuity of< 0.1 after surgery, there were 0cases in the observation group, there were 4 cases in the control group, the difference was no statistically significant( χ^(2) =5.801, P< 0.05).Conclusion In the treatment of patients with acute angle-closure glaucoma,scleral tunnel incision trabeculectomy can effectively and stably reduce intraocular pressure,and improve the patient’s vision, and the curative effect is remarkable worthy of populari
