化学发光免疫法检测血清肿瘤标志物对乳腺癌的诊断价值

摘　　要：目的探讨化学发光免疫法检测血清肿瘤标志物对乳腺癌的诊断价值。方法选取佛山市妇幼保健院2019年12月至2021年4月收治的乳腺恶性肿瘤患者86例作为观察1组,良性肿瘤患者84例作为观察2组,选择同期进行体检且结果显示健康的女性90例作为对照组。对所有受试者采用化学发光免疫法检测血清肿瘤标志物,并比较3组受试者血清癌胚抗原(CEA)、癌抗原15-3(CA15-3)水平。结果观察1组CEA、CA15-3水平及阳性例数均显著高于对照组以及观察2组,差异有统计学意义(P<0.05);观察1组治疗前后CEA、CA15-3水平比较差异有统计学意义(P<0.05);单独检测CEA的敏感度为34.88%、特异性为96.43%以及准确度为65.29%;单独检测CA15-3的敏感度为46.51%、特异性为94.43%以及准确度为70.00%;联合检测的敏感度为81.40%、特异性为90.48%以及准确度为85.88%。结论采用化学发光免疫法检测血清肿瘤标志物对乳腺癌的诊断价值较高,将CEA、CA15-3联合进行检测能够提升临床对于乳腺癌诊断的有效性以及敏感度。Objective To explore the diagnostic value of chemiluminescence immunoassay for serum tumor markers in breast cancer.Methods 86 cases of breast cancer patients admitted to Foshan Maternal and Child Health Care Hospital from December 2019 to April 2021 were selected as observation group 1,84 cases of benign tumor patients as observation group 2,and 90 cases of healthy women who underwent physical examination at the same time were selected as control group.Chemiluminescence immunoassay was used to detect serum tumor markers for all subjects,and the serum levels of carcinoembryonic antigen(CEA)and carcinoembryonic antigen 15-3(CA15-3)were compared among the three groups of subjects.Results CEA,CA15-3 levels and positive cases in observation group 1 were significantly higher than those in control group and observation group 2(P<0.05);The levels of CEA and CA15-3 in group 1 were statistically significant before and after treatment(P<0.05);The sensitivity,specificity and accuracy of CEA were 34.88%,96.43%and 65.29%respectively;The sensitivity,specificity and accuracy of CA15-3 were 46.51%,94.43%and 70.00%respectively;The sensitivity,specificity and accuracy of the combined assay were 81.40%,90.48%and 85.88%respectively.Conclusion Chemiluminescence immunoassay has a high diagnostic value for serum tumor markers in breast cancer,and the combined detection of CEA and CA15-3 can improve the effectiveness and sensitivity of clinical diagnosis of breast cancer.
