聚乙二醇化重组人粒细胞刺激因子在乳腺癌化疗中的应用效果分析

摘　　要：目的 分析聚乙二醇化重组人粒细胞刺激因子在乳腺癌化疗中的应用效果。方法 选取新疆医科大学第一附属医院乳腺外科2018年1月-2021年11月因乳腺癌行化学治疗的1 024例患者作为研究对象,根据化疗后是否预防性应用聚乙二醇化重组人粒细胞刺激因子(PEG-rh G-CSF)分为2组,518例不应用PEG-rh G-CSF(对照组),506例应用PEG-rh G-CSF(观察组),比较2组患者化疗后Ⅲ~Ⅳ级骨髓抑制、发热、感染发生率,静脉抗生素应用率,再住院率及化疗药物减量或延迟率。结果 观察组Ⅲ~Ⅳ级骨髓抑制、发热及感染等不良反应发生率,再住院率,静脉抗生素应用率,化疗药物减量或延迟率均较对照组明显降低,差异有统计学意义(P<0.05)。结论PEG-rh G-CSF的预防性应用能够显著降低乳腺癌化疗后Ⅲ~Ⅳ级骨髓抑制、发热、感染、再住院、静脉抗生素应用及化疗药物减量或延迟等发生率,对减轻患者化疗后身心创伤、保障化疗周期的足剂量续贯进行及优化乳腺癌管理具有重要的临床应用价值。Objective To retrospectively analyze and evaluate the actually efficacy and value of pegylated Recombinant Human Granulocyte Colony-stimulating Factor in the management of breast cancer after chemotherapy.Methods A total of1024 patients with breast cancer who received chemotherapy from January 2018 to November 2021 in the Department of breast surgery,the First Affiliated Hospital of Xinjiang Medical University were selected as the research objects. According to whether PEG-rh G-CSF was used prophylactically after chemotherapy,they were divided into two groups. 518 patients in the control group were not treated with PEG-rh G-CSF,while 506 patients in the observation group were treated with PEG-rh G-CSF. The incidence of grade Ⅲ ~ Ⅳ myelosuppression,fever,infection,intravenous antibiotic use rater,rehospitalization rate and reduction or delay rate of chemotherapy drugs were compared between the two groups.Results The incidence of adverse reactions such as grade III-IV bone marrow suppression,fever and infection,the application rate of intravenous antibiotics,the rehospitalization rate and the reduction or delay rate of chemotherapy drugs in the observation group were significantly lowerer than those in the control group,The difference was statistically significant(P<0.05).Conclusion The preventive application of PEG-rh G-CSF can significantly reduce the incidence of Ⅲ~Ⅳ level myelosuppression,fever,infection,rehospitalization,intravenous antibiotic application and reduction or delay rate of chemotherapy drugs after breast cancer chemotherapy,which have practical and effective clinical application value in reducing the physical and psychological trauma of patients after chemotherapy,ensuring the continuity of chemotherapy cycle and optimizing themanagement of breast cancer.
