促甲状腺激素 甲状腺球蛋白及血管内皮生长因子联合检测对甲状腺癌术后转移病情评估的意义

摘　　要：目的 通过促甲状腺激素(TSH)、甲状腺球蛋白(Tg)及血管内皮生长因子(VEGF)在甲状腺癌术后患者血清中的检测,探讨三者联合检测在甲状腺癌术后患者中的应用价值。方法 选取2020年1月至2021年12月106例甲状腺癌术后患者(32例局部转移组、40例未转移组、34例肺转移组)、33名健康体检者(对照组)作为本研究对象,检测其血清TSH、Tg、VEGF的水平,同时分析其相关性。结果 TSH、Tg、VEGF水平均为甲状腺癌肺转移组>局部转移组>未转移组(P<0.05),三者水平在未转移组与对照组中的比较,差异无统计学意义(P>0.05)。甲状腺癌转移组血清VEGF水平与血清Tg水平呈正相关(P<0.01)。TSH、Tg、VEGF的曲线下面积(AUC)分别为0.743、0.808、0.819。结论 血清TSH、Tg、VEGF水平在甲状腺癌术后转移患者血清中均升高,三者联合检测对于甲状腺癌术后转移患者的病情评估意义重大。Objective To measure the serum levels of thyroid stimulating hormone(TSH),thyroglobulin(Tg) and vascular endothelial growth factor(VEGF) in postoperative patients with thyroid cancer,and to determine the value of panel detection of these indicators among these patients. Methods A total of 106postoperative thyroid cancer patients(including 32 with local metastasis, 40 without metastasis and 34 with pulmonary metastasis)and 33 healthy physical checkup subjects(control group)in our hospital between January2020 and December 2021 were included in this study. For these subjects,the serum levels of TSH,Tg,and VEGF were detected and analyzed for any potential correlations. Results The levels of TSH,Tg and VEGF were the highest in the lung-metastasis group,followed by local metastasis group and non-metastasis group(P<0.05),but were not statistically different between the non-metastasis group and the control group(P >0.05). There was a positive linear correlation between the serum VEGF and Tg levels in the thyroid cancer patients with any metastasis(P<0.01). The area under the curve(AUC)of TSH,Tg and VEGF were 0.743,0.808 and 0.819,respectively. Conclusion The serum levels of TSH, Tg and VEGF are elevated in thyroid cancer patients with postoperative metastasis. Panel detection of the three indicators can be importantly significant for assessment of postoperative metastasis in patients with thyroid cancer.
