螺旋CT多平面重组技术在喉癌诊断中的应用有效性及价值研究  被引量:2

摘  要:目的分析在喉癌患者诊断中螺旋CT多平面重组技术(MPR)的有效性与价值。方法遴选本院治疗的57例喉癌患者为对象实施研究,研究起止时间段为2019年11月~2020年11月,对于全部患者均实施术前CT检查,同时进行MPR后处理,以术后病理切片病变范围为标准,分析MPR、横断面、MPR图像联合横断面的诊断结果,应用“χ^(2)”实施统计学处理。结果57例喉癌中,26例声门型,4例声门癌,27例声门上型癌;在喉内结构受侵诊断中,联合图像诊断会厌的准确性为96.5%,杓会厌皱襞准确性为88.6%,室带准确性为94.26%,声带准确性为82.6%,前联合准确性为91.8%。前联合诊断中,联合图像优于MPR图像,差异存在统计学价值,P<0.05;在喉外结构受侵诊断中,联合图像诊断会厌前间隙的准确性为100.0%,喉旁间隙准确性为94.7%,梨状窝准确性为94.5%。喉旁间隙诊断中,联合图像优于MPR图像,差异存在统计学价值,P<0.05。结论在喉癌的诊断中,螺旋CT可有效显示出肿瘤大小、形态、侵犯范围、颈部淋巴结转移情况,MPR可更加直观地显示出肿瘤范围与形态,可对常规轴面图像进行补充,联合MPR与横断面图像,可进一步提高喉癌的诊断准确性,故MPR具有理想的应用价值。Objective To analyze the effectiveness and value of spiral CT multiplanar reconstruction(MPR)in the diagnosis of patients with laryngeal cancer.Methods The 57 patients with laryngeal cancer treated in our hospital were selected for the study.The study period was from November 2019 to November 2020.All patients were subjected to preoperative CT examination,and MPR post-processing was performed at the same time.The pathological section of the posterior pathological section is the standard,and the diagnosis results of MPR,cross-section,and MPR image combined with cross-section are analyzed,and statistical processing is performed using“χ^(2)”.Results Among 57 cases of laryngeal cancer,26 cases were glottal type,4 cases were glottic cancer,and 27 cases were supraglottic type cancer.In the diagnosis of intralarynx structure invasion,the accuracy of combined image diagnosis of epiglottis was 96.5%,and dipper epiglottis.The accuracy of folds was 88.6%,the accuracy of ventricular cords was 94.26%,the accuracy of vocal cords was 82.6%,and the previous joint accuracy was 91.8%.In the anterior joint diagnosis,the joint image is better than the MPR image,and the difference is statistically significant,P<0.05;in the diagnosis of the extralaryngeal structure invasion,the accuracy of the joint image in the diagnosis of the anterior epiglottis space is 100.0%,and the accuracy of the paralarynx space is 94.7%,the accuracy of pyriform fossa was 94.5%.In the diagnosis of the paralarynx space,the combined image is better than the MPR image,and the difference is statistically significant,P<0.05.Conclusion In the diagnosis of laryngocarcinoma,spiral CT can effectively show the tumor size,shape,invasion range,and cervical lymph node metastasis.MPR can more intuitively show the tumor range and shape,which can supplement conventional axial images.Combining MPR and cross-sectional images can further improve the diagnostic accuracy of laryngeal cancer,so MPR has ideal application value.
