从风痰论定痫丸加减治疗脑胶质瘤验案

摘　　要：通过探讨总结郑玉玲教授从风痰辨治脑胶质瘤的临证经验,以期为临床治疗脑胶质瘤提供参考借鉴。郑玉玲教授认为脑胶质瘤的发病与风、火、痰、瘀、虚等致病因素紧密相关,提出“风痰上扰、正气亏虚”为其核心病机。上扰之风痰具有胶黏难化、挟瘀固结、症状纷杂、侵及肝脾肾的致病特点。在治疗中常将手术前后作为脑胶质瘤攻实补虚侧重的分界线,但不可拘泥章法,墨守陈规。根据脑胶质瘤的病因病机,郑玉玲教授临床常以定痫丸为基础方,先定祛风化痰之法,后审气血阴阳之偏颇,根据患者主症之不同,病机证素之变化,临证加减配伍,并具体分析了方中引经药、通腑药、开窍药以及虫类药的用药特色。By discussing and summarizing professor ZHENG Yu-ling’s clinical thoughts and experience in treating glioma from wind-phlegm,to provide a reference for clinical treatment of glioma.Professor ZHENG Yu-ling believes that the onset of glioma is closely related to wind,fire,phlegm,blood stasis,deficiency and other pathogenic factors,and puts forward that‘the disturbance of wind-phlegm and the deficiency of vital qi’as its core pathogenesis.The wind-phlegm of the upper disturbance has the pathogenic characteristics of stickiness,difficult to resolve,stasis consolidation,complicated symptoms,and invasion of liver,spleen and kidney.In the treatment,it can often be used as the boundary line for glioma to focus on attacking solid or tonifying deficiency before and after an operation,but it should not stick to rules and regulations.According to the pathogenesis of glioma,professor ZHENG Yu-ling often takes Dingxian Pill as the basic prescription in clinical practice.Professor ZHENG Yuling first determined the method of expelling wind and phlegm,and then reviewed the bias of qi,blood,yin and yang.According to the different main symptoms of the patients,the changes of pathogenesis and syndrome elements,and the compatibility of addition and subtraction in clinical syndromes,the characteristics of the medicines of meridian-inducing,Tongfu,resuscitation and insect medicines were analyzed in detail.
