7种肺癌自身抗体的临床检测意义

摘　　要：目的探究这7种自身抗体:抑癌基因53(P53)、蛋白基因产物9.5(PGP9.5)、性别决定基因家族2(SOX2)、肿瘤/睾丸抗原G抗原7(GAGE7)、ATP结合RNA解旋酶(GBU4-5)、黑色素瘤抗原A1(MAGEA1)、肿瘤相关基因(CAGE)在肺癌中的临床检测意义。方法回顾性分析2019年2月至2020年2月本院180例初诊肺癌的患者和80例体检健康者血清临床资料。采用酶联免疫吸附法(ELISA)分别检测肺癌患者(观察组)、健康者(对照组)血清P53、PGP9.5、SOX2、GAGE7、GBU4-5、MAGEA1、CAGE的抗体水平;不同病理类型肺癌患者7种自身抗体检测的阳性率比较;评估7种自身抗体联合检测的诊断效能。结果观察组患者血清P53、PGP9.5、SOX2、GAGE7、GBU4-5、MAGEA1、CAGE水平均高于对照组(P<0.05);鳞癌组血清P53、PGP9.5、SOX2、GAGE7、GBU4-5、MAGEA1、CAGE均高于腺癌组(P<0.05);7种自身抗体联合检测诊断初诊肺癌患者敏感性为73.4%,特异性为93.1%,AUCROC为0.930,其特异性和AUCROC均高于各自身抗体单项检测。结论这7种肺癌自身抗体的实验室检测为肺癌早期诊断提供了可靠的诊断依据,有一定的临床应用价值。Objective To explore the clinical significance of these seven autoantibodies:tumor suppressor gene 53(p53),protein gene product 9.5(PGP9.5),sex determining gene family 2(Sox2),tumor/testicular antigen G antigen 7(GAGE7),ATP binding RNA helicase(gbu4-5),melanoma antigen A1(MAGEA1),tumor associated gene(cage)in lung cancer.Methods The serum clinical data of 180 patients with lung cancer and 80 healthy people in our hospital from February 2019 to February 2020 were retrospectively analyzed.Enzyme linked immunosorbent assay(ELISA)was used to detect the serum antibody levels of p53,PGP9.5,Sox2,GAGE7,gbu4-5,MAGEA1 and cage in patients with lung cancer(observation group)and healthy people(control group);The positive rates of 7 kinds of autoantibodies in patients with different pathological types of lung cancer were compared;To evaluate the diagnostic efficacy of combined detection of seven autoantibodies.Results The serum levels of p53,PGP9.5,Sox2,GAGE7,gbu4-5,MAGEA1 and cage in the observation group were higher than those in the control group(P<0.05);The levels of serum p53,PGP9.5,Sox2,GAGE7,gbu4-5,MAGEA1 and cage in squamous cell carcinoma group were higher than those in adenocarcinoma group(P<0.05);The sensitivity,specificity and aucroc of the combined detection of seven autoantibodies were 73.4%,93.1%and 0.930,respectively.Conclusion The laboratory detection of these seven kinds of lung cancer autoantibodies provides reliable diagnostic basis for early diagnosis of lung cancer,and has certain clinical application value.
