摘  要:目的探讨调节过程中儿童眼与老视眼不同部位睫状肌厚度的变化。方法纳入志愿者共37例,分为儿童组(21例42眼)及老视组(16例32眼)。采用眼前段光学相干断层成像(OCT)分别对调节前后的两组巩膜突后1、2、3 mm(CM1、CM2、CM3)处睫状肌厚度及垂直于巩膜面的睫状肌最大厚度(CMm)进行测量,比较两组睫状肌厚度的变化。结果两组调节状态及非调节状态比较,在CM1及CMm处差异有统计学意义(P<0.01),在CM2、CM3处差异无统计学意义(P>0.05);调节前后睫状肌厚度的变化中,两组均表现为CMm、CM1增厚,CM3变薄(P<0.05);儿童组调节前后CM2变薄(P<0.05),老视组在CM2无明显变化(P>0.05)。结论调节时前部睫状肌增厚,前部睫状肌的收缩在调节中起主要作用;相比于儿童眼,老视眼的前部睫状肌厚度变薄,前部睫状肌厚度的下降可能与老视的形成相关。Objective To explore the changes in the ciliary muscle thickness at different parts between children′s eyes and presbyopia during accommodation.Methods A total of 37 volunteers were divided into a children′s group(n=21 with 42 eyes)and a presbyopia group(n=16 with 32 eyes).Anterior segment optical coherence tomography(OCT)was used to measure the ciliary muscle thickness at 1,2,and 3 mm(CM1,CM2,CM3)posterior to the scleral process and the maximum ciliary muscle thickness(CMm)perpendicular to the scleral surface before and after adjustment.The changes of ciliary muscle thickness were compared between the two groups.Results There was a significant difference in CM1 and CMm between the two groups in accommodation and non-accommodation(P<0.01),but there was no significant difference in CM2 and CM3(P>0.05).In the changes of ciliary muscle thickness before and after adjustment,both groups showed thickening of CMm and CM1,and thinning of CM3(P<0.05).In the children′s group,the CM2 became thinner and had statistical difference before and after adjustment(P<0.05),whilethe presbyopia group had no significant change in CM2 and had no statistical difference(P>0.05).Conclusions The anterior ciliary muscle is thickened during accommodation.The contraction of the anterior ciliary muscle plays a major role in accommodation.Compared with children′s eyes,the anterior ciliary muscle thickness of presbyopia is thinning.The decrease of anterior ciliary muscle thickness may be related to the formation of presbyopia.
