摘　　要：目的探究黏膜组织叉头框蛋白A2(FOXA2)、低氧诱导因子-1α(HIF-1α)表达水平与溃疡性结肠炎(UC)患者疾病严重程度、预后的关系。方法选取活动期UC患者74例为UC组,按Truelove-Witts评分标准将UC患者分为重度UC组(22例)、中度UC组(27例)、轻度UC组(25例),根据2年随访情况将UC患者分为预后良好组(43例)和预后不良组(31例);另纳入同期进行结肠镜检查的健康者28例为对照组。测定黏膜组织FOXA2、HIF-1αmRNA表达水平;分析UC患者黏膜组织FOXA2 mRNA表达水平与HIF-1αmRNA的相关性;比较不同严重程度UC患者黏膜组织FOXA2、HIF-1αmRNA表达水平;比较不同预后UC患者黏膜组织FOXA2 mRNA、HIF-1αmRNA表达水平。结果UC组患者黏膜组织FOXA2 mRNA表达水平低于对照组(0.52±0.17 vs.1.09±0.36,t=10.848,P<0.001),HIF-1αmRNA表达水平高于对照组(1.67±0.56 vs.0.98±0.33,t=6.199,P<0.001);UC患者黏膜组织FOXA2 mRNA表达水平与HIF-1αmRNA呈负相关(r=-0.597,P<0.05);重度UC组患者黏膜组织FOXA2 mRNA表达水平(0.26±0.09)低于中度UC组(0.54±0.18)(P<0.05),HIF-1αmRNA表达水平(2.18±0.73)高于中度UC组(1.64±0.55)(P<0.05);中度、重度UC组患者黏膜组织FOXA2 mRNA表达水平低于轻度UC组(0.71±0.24)(P<0.05),HIF-1αmRNA表达水平高于轻度UC组(1.25±0.42)(P<0.05);预后不良组UC患者黏膜组织FOXA2 mRNA表达水平(0.30±0.10)低于预后良好组(0.68±0.23)(t=8.618,P<0.001),HIF-1αmRNA表达水平(2.06±0.69)高于预后良好组(1.39±0.46)(t=5.013,P<0.001)。结论UC患者黏膜组织FOXA2 mRNA表达水平较低,HIF-1αmRNA表达水平较高,检测黏膜组织FOXA2、HIF-1αmRNA水平有助于临床判定UC患者病情和预后情况。Objective To evaluate the correlation between the expression levels of forkhead box protein A2(FOXA2)and hypoxia-inducible factor-1α(HIF-1α)in mucosal tissues and the severity or prognosis of ulcerative colitis(UC).Methods Seventy-four patients with active UC were gathered as the UC group.According to the Truelove-Witts scoring standard,UC patients were divided into severe UC group(22 cases),moderate UC group(27 cases),and mild UC group(25 cases).According to the 2-year follow-up,UC patients were divided into good prognosis group(43 cases)and poor prognosis group(31 cases).Meanwhile,28 healthy people who received colonoscopy were included as the control group.The expression levels of FOXA2 and HIF-1αmRNA in mucosal tissues were measured.The correlation between FOXA2 mRNA expression level and HIF-1αmRNA in mucosal tissues of UC patients was analyzed.The mRNA expression levels of FOXA2 and HIF-1αin mucosal tissues were compared among UC patients with different severity.The expression levels of FOXA2 mRNA and HIF-1αmRNA in mucosal tissues of UC patients with different prognosis were also compared.Results The expression level of FOXA2 mRNA in mucosal tissues of the UC group was significantly lower than that of the control group(0.52±0.17 vs.1.09±0.36,t=10.848,P<0.001),and the expression level of HIF-1αmRNA was signficanlty higher than that of the control group(1.67±0.56 vs.0.98±0.33,t=6.199,P<0.001).The expression level of FOXA2 mRNA in mucosal tissues of UC patients was significantly negatively correlated with HIF-1αmRNA(r=-0.597,P<0.05).The expression level of FOXA2 mRNA in the mucosal tissue of the severe UC group(0.26±0.09)was significantly lower than that of the moderate UC group(0.54±0.18)(P<0.05),and the expression level of HIF-1αmRNA(2.18±0.73)was significantly higher than that of the moderate UC group(1.64±0.55)(P<0.05).The expression level of FOXA2 mRNA in mucosal tissues of moderate and severe UC group was significantly lower than that of mild UC group(0.71±0.24)(P<0.05),and the expressio
