摘  要:目的分析脑白质疏松症(LA)与血清C肽水平(CP)、认知功能的关系。方法选择脑白质疏松症患者128例,按照严重程度分为轻度组(n=40)、中度组(n=52)、重度组(n=36),收集3组临床资料,比较3组血清C肽水平和蒙特利尔认知评价量表(MoCA)评分。结果3组HDL-C水平比较,轻度组>中度组>重度组;IMT比较,轻度组<中度组<重度组(均P<0.05)。3组血清C肽水平比较,轻度组>中度组>重度组;MoCA评分比较,轻度组>中度组>重度组(均P<0.05)。Spearman相关分析结果显示,脑白质疏松症程度与血清C肽水平(r=-0.831,P<0.001)、MoCA评分(r=-0.830,P<0.001)均呈负相关。HDL-C、血清C肽水平均是影响脑白质疏松症严重程度的独立影响因素。HDL-C诊断脑白质疏松症的AUC值为0.755、灵敏度为53.3%、特异性为88.9%;血清C肽诊断脑白质疏松症的AUC值为0.935、灵敏度为82.6%、特异性为88.9%。结论脑白质疏松症与血清C肽水平呈负相关,随着脑白质疏松症程度加深、血清C肽水平下降、认知功能下降,血清C肽是影响脑白质疏松症严重程度的独立危险因素,且对脑白质疏松症具有较好的诊断效能。Objecive To analyze the correlation between leukoaraiosis(LA),serum C-peptide level(CP)and cognitive function.Methods A total of 128 patients with leukmatter admitted to our hospital from January 2019 to January 2021 were divided into a mild group(n=40),a moderate group(n=52)and a severe group(n=36)according to their illness severity.The clinical data of the three groups were collected,and the level of serum C-peptide and the score of the Montreal Cognitive Assessment Scale(MoCA)were compared between the three groups.Results In terms of the HDL-C level,the mild group was the highest,followed by the moderate group and the severe group,while in terms of the IMT level,the mild group was the lowest,followed by the moderate group and the severe group(all P<0.05).With respect to the serum C-peptide level and the MoCA score of the three groups,the mild group was the highest,followed by the moderate group and the severe group(all P<0.05).The Spearman correlation analysis showed that the degree of LA was negatively correlated with the serum C-peptide level(r=-0.831,P<0.001)and the MoCA score(r=-0.830,P<0.001).The HDL-C and serum C peptide levels were both independent factors affecting the LA severity.The AUC value of HDL-C in diagnosing LA was 0.755,with a sensitivity 53.3%and a specificity 88.9%.The AUC value of serum C-peptide in diagnosing LA was 0.935,with a sensitivity 82.6%and a specificity 88.9%.Conclusion There is a negative correlation between leukoaraiosis and the serum C-peptide level,and as the degree of the disease increases,the serum C-peptide level and the cognitive function both decline.Therefore,serum C-peptide is an independent risk factor affecting the severity of LA,which has a good diagnostic effect for LA.
