白内障超声乳化联合房角分离术治疗急性原发性闭角型青光眼的疗效观察

摘　　要：目的观察白内障超声乳化联合房角分离术治疗急性原发性闭角型青光眼的临床效果。方法选取2019年1月至2021年4月蚌埠医学院第一附属医院收治的急性原发性闭角型青光眼患者48例,其中男性17例,女性31例,随机分为实验组24例(26只眼)和对照组24例(25只眼),实验组接受白内障超声乳化联合房角分离术,对照组接受白内障超声乳化联合小梁切除术,观察临床效果及术前术后各项临床指标。结果所有的患者随访周期为(12.00±4.78)个月,两组患者术前视力、眼压、中央前房深度、前房角的开放程度、眼轴方面比较差异无统计学意义(P>0.05);术后1 d、1周、1个月眼压实验组高于对照组,两组比较差异具有统计学意义(P<0.01);术后3个月、6个月、12个月两组眼压比较无明显差异(P>0.05);两组患者中央前房深度、前房角的开放程度术前与术后1年比较差异具有统计学意义(P<0.01),实验组优于对照组(P<0.01);实验组眼轴术前与术后1年比较无明显差异,而对照组差异明显(P<0.01);两组患者术后1年视力改善效果比较差异无统计学意义(P>0.05)。结论白内障超声乳化联合房角分离术治疗急性原发性闭角型青光眼治疗效果显著,能够有效降低眼压,提高视力,且微创可控,值得临床推广应用。Objective To observe the clinical effect of phacoemulsification combined with goniosynechialysis on acute primary angle-closure glaucoma.Methods This study selected 48 patients with acute primary angle-closure glaucoma admitted to The First Affiliated Hospital of Bengbu Medical College from January 2019 to April 2021,including 17 males and 31 females.They were randomly divided into an experimental group(n=24,26 eyes)and a control group(n=24,25 eyes).The experimental group were treated with phacoemulsification combined with goniosynechialysis,and the control group received phacoemulsification combined with trabeculectomy.The researchers observed the clinical effects and relevant clinical indicators before and after surgery.Results All the patients were followed up for(12.00±4.78)months.There were no significant differences in preoperative visual acuity,intraocular pressure,central anterior chamber depth,anterior chamber angle openness,and ocular axis between two groups(P>0.05).The experimental group had higher intraocular pressure at 1 day,1 week and 1 month after operation than the control group,and the differences were statistically significant(P<0.01).There was no significant difference in intraocular pressure between two groups at 3 months,6 months and 12 months after operation(P>0.05).There were significant differences in central anterior chamber depth and anterior chamber angle openness between two groups before the surgery and 1 year after surgery(P<0.01),in terms of which the experimental group was better than the control group(P<0.01).No significant difference was found in ocular axis of the experimental group before operation and at 1 year after operation,while the control group had significant difference in terms of such comparison(P<0.01).There was no significant difference in visual acuity improvement between two groups at 1 year after operation(P>0.05).Conclusion Phacoemulsification combined with goniosynechialysis in the treatment of acute primary angle-closure glaucoma has a significant effect.Thi
