摘  要:【目的】探讨纳米结构涂层密封剂联合抗菌密封剂对种植体牙周病原菌及骨密度的影响。【方法】选择2018年1月至2020年1月在本院接受下颌后牙区种植的190例患者,根据入院顺序编号随机分为对照组(不使用封闭剂)、常规组(使用种植体密封胶)、纳米组(使用纳米结构涂层密封剂)、抗菌组(使用抗菌密封胶)、联合组(使用纳米结构涂层密封+密封胶),每组38例。从治疗起随访至术后12个月,比较各组种植体稳定性、种植体成功率、牙周病原菌及骨密度。【结果】粘接即刻及种植体加载后3个月、6个月、12个月,对照组、常规组、抗菌组、纳米组、联合组粘合稳定性系数(ISQ)值依次增高,组间比较差异有统计学意义(P<0.05),且粘接即刻各组ISQ值高于种植体加载后(P<0.05)。随着加载时间延长,对照组、常规组、纳米组、抗菌组、联合组金黄色葡萄球菌、口腔内固有血链球菌、牙龈卟啉单胞菌数量增加,各组种植体加载后3个月、6个月、12个月与粘接即刻比较,差异有统计学意义(P<0.05),且联合组病原菌数量增加最低。种植体加载后6个月、12个月,联合组骨密度大于纳米组、抗菌组,纳米组骨密度大于抗菌组,差异有统计学意义(P<0.05)。【结论】纳米结构涂层密封剂联合抗菌密封胶可改善种植体牙周病原菌及骨密度情况,提高种植体稳定性,值得临床推广应用。【Objective】To investigate the effect of nanostructured coating combined with antiseptic sealant on periodontal pathogens and bone density of implants.【Methods】A total of 190 patients with mandibular implants in our hospital from January 2018 to January 2020 were enrolled,and divided into five groups according to admission orders,each with 38 cases.Control group treated without sealant,routine group treated with implant sealant,nano group treated with nanostructured coating,antiseptic group treated with antiseptic sealant,and combined group received nanostructured coating combined with antiseptic sealant.Patients were followed up from treatment till post-treatment 12 months.Then the implant stability,implant success rate,periodontal pathogens and bone density were compared among groups.【Results】At the moment of bonding and 3 months,6 months,and 12 months after loading,the implant stability quotient(ISQ)values from low to high were the control group,the conventional group,the antibacterial group,the nano group,and the combined group,with statistical differences(P<0.05),and the isq values at the moment of bonding in each group were higher than those after loading(P<0.05).With the extension of loading time,the number of Staphylococcus aureus,oral inherent Streptococcus sanguis and Porphyromonas gingivalis in the control group,routine group,nano group,antibacterial group and combined group increased.There was a statistical difference between the number of Staphylococcus aureus,oral inherent Streptococcus sanguis and Porphyromonas gingivalis 3 months,6 months and 12 months after loading and immediately after bonding(P<0.05).The increase of pathogenic bacteria in the combined group was the lowest and lower than that in the other four groups(P<0.05).At 6 and 12 months after loading,the bone mineral density of the combined group was higher than that of the nano group and the antibacterial group,and the bone mineral density of the nano group was higher than that of the antibacterial group,the difference was sta
