摘 要:【目的】探讨数字减影血管造影(DSA)与钆贝葡胺增强磁共振成像(MRI)对肝癌(HCC)经导管动脉化疗栓塞(TACE)术后复发微小病灶的诊断效能。【方法】选取2017年1月至2021年1月本院收治的85例行TACE术治疗的HCC患者,所有患者均行DSA、钆贝葡胺增强MRI检查,以病理检查结果作为微小肝癌病灶复发结果。记录DSA、钆贝葡胺增强MRI诊断微小肝癌病灶复发结果,分析两种诊断方法的效能。【结果】TACE术后,85例HCC患者中有41例(48.2%)患者经穿刺病理检查证实为复发微小病灶。DSA、钆贝葡胺增强MRI诊断分别显示38例、39例为复发微小病灶。DSA诊断复发微小病灶的灵敏度、特异度分别为92.7%、100.0%,钆贝葡胺增强MRI诊断复发微小病灶的灵敏度、特异度分别为95.1%、100.0%,差异无统计学意义(P>0.05)。DSA、钆贝葡胺增强MRI诊断与病理诊断的一致性Kappa值分别为0.791、0.778,DSA与钆贝葡胺增强MRI诊断的一致性Kappa值为0.802。【结论】DSA、钆贝葡胺增强MRI诊断肝癌TACE术后复发微小病灶效能相似,可根据个体情况合理选择适合诊断方法。【Objective】To investigate the diagnostic efficacy of digital subtraction angiography(DSA)and gadolinium enhanced magnetic resonance imaging(MRI)in the recurrence of small lesions after transcatheter arterial chemoembolization(TACE)of liver cancer.【Methods】A total of 85 HCC patients treated with TACE in our hospital from January 2017 to January 2021 were selected.All patients were examined with DSA and gadolinium enhanced MRI,and the pathological results were taken as the recurrence results of small liver cancer.The results of DSA and gadolinium enhanced MRI in the diagnosis of recurrence of small liver cancer were recorded,and the efficacy of the two diagnostic methods was analyzed.【Results】After TACE,41 out of 85 HCC patients(48.2%)were confirmed to have relapsed small lesions by biopsy.DSA and gadolinium bemeglumine enhanced MRI showed that 38 cases and 39 cases were small recurrent lesions,respectively.The sensitivity and specificity of DSA were 92.7%and 100.0%respectively for the diagnosis of recurrent small lesions,and the sensitivity and specificity of gadolinium bemeglumine enhanced MRI for the diagnosis of recurrent small lesions were 95.1%and 100.0%respectively,with no significant difference(P>0.05).The consistency between DSA and gadolinium bemeglumine enhanced MRI diagnosis and pathological diagnosis was 0.791 and 0.778 respectively,and the consistency between DSA and gadolinium bemeglumine enhanced MRI diagnosis was 0.802.【Conclusion】DSA and gadolinium bemeglumine enhanced MRI have similar efficacy in the diagnosis of small recurrent lesions of hepatocellular carcinoma after TACE,and suitable diagnostic methods can be selected reasonably according to individual conditions.
