摘 要:目的分析128排螺旋CT增强扫描对于鉴别肺孤立性结节良恶性的临床价值。方法80例肺孤立性结节患者,均接受128排螺旋CT增强扫描,分析患者肺孤立性结节形态特点。术后以病理学检查结果为金标准,分析128排螺旋CT增强扫描对于肺孤立性结节良恶性的诊断效能。结果80例孤立性结节患者,经128排螺旋CT扫描后其中存在恶性结节63例,其中34例患者存在毛刺征,占比53.97%,存在分叶不规则征23例,占比36.51%,存在胸膜凹陷征16例,占比25.40%,存在边界模糊12例,占比19.05%,存在空泡征10例,占比15.87%。经临床病理学检查得知,恶性结节患者为64例,其中肺腺癌60例,肺鳞癌2例,肺小细胞癌2例;良性结节患者16例,其中5例结核,6例肺囊肿,5例错构瘤。128排螺旋CT增强扫描对肺孤立性结节的灵敏度为96.87%(62/64),特异度为93.75%(15/16),准确率为96.25%(77/80),阳性预测值为98.41%(62/63),阴性预测值为88.24%(15/17)。结论术前接受128排螺旋CT增强扫描能够有效鉴别孤立性结节良恶性,提升诊断准确率、灵敏度、特异度,为后续诊断及治疗奠定基础。Objective To analyze the clinical value of 128-slice spiral CT enhanced scanning in differentiating benign and malignant solitary pulmonary nodules. Methods 80 patients with isolated pulmonary nodules underwent 128-slice spiral CT enhancement scanning, and the morphological features of the patients’ isolated pulmonary nodules were analyzed. Postoperatively, the diagnostic efficacy of 128-slice spiral CTenhanced scan for the diagnosis of benign and malignant isolated pulmonary nodules was analyzed, using pathological findings as the gold standard. Results In the present study, 80 patients with isolated nodules were scanned by 128-slice spiral CT, of which 63 patients had malignant nodules, of which 34 patients had burr sign,accounting for 53.97%, 23 patients had lobulated irregular sign, accounting for 36.51%, 16 patients had pleural indentation sign, accounting for 25.40%, 12 patients had blurry border, accounting for 19.05%, and 10 patients had vacuole sign, accounting for 15.87%. The clinicopathological examination revealed that there were 64 patients with malignant nodules, including 60 cases of lung adenocarcinoma, 2 cases of lung squamous carcinoma, and 2 cases of small cell carcinoma;there were 16 patients with benign nodules, including 5 cases of tuberculosis,6 cases of pulmonary cysts, and 5 cases of hamartoma. The sensitivity, specificity, accuracy, positive prediction and negative prediction of 128-slice spiral CT enhanced scanning for the diagnosis of solitary pulmonary nodules were 96.87%(62/64), 93.75%(15/16), 96.25%(77/80), 98.41%(62/63) and 88.24%(15/17) respectively.Conclusion Preoperative 128-slice spiral CT scanning can effectively identify benign and malignant isolated nodules, improve diagnostic accuracy, sensitivity and specificity, and lay the foundation for subsequent diagnosis and treatment.
