摘　　要：目的探讨中性粒细胞与淋巴细胞比值(NLR)对前循环大血管闭塞缺血性脑卒中患者短期预后的预测价值。方法选取2018年1月~2021年3月于郑州市第一人民医院住院的93例老年前循环大血管闭塞急性缺血性脑卒中患者进行回顾性分析,根据脑卒中3个月后改良Rankin评分(mRS)分为预后良好组(mRS≤2分)和预后不良组(mRS>2分),收集临床资料进行统计学分析。结果单因素分析显示,预后不良组白细胞计数、中性粒细胞比例、NLR及入院美国国立卫生研究院脑卒中量表(NIHSS)评分高于预后良好组,预后不良组红细胞计数低于预后良好组,差异均有统计学意义(P<0.05)。多因素logistic回归分析结果显示,入院NIHSS评分、NLR是前循环大血管闭塞急性缺血性脑卒中患者短期不良预后的独立危险因素(P<0.05)。受试者工作特征曲线分析显示,NLR预测前循环大血管闭塞急性缺血性脑卒中患者短期不良预后的曲线下面积为0.852(95%CI:0.778~0.926,P<0.001)。结论NLR对前循环大血管闭塞缺血性脑卒中患者的短期预后具有一定预测价值。Objective To explore the predictive value of neutrophil-lymphocyte ratio(NLR)for short-term prognosis in patients with ischemic stroke due to anterior circulation large vessel occlusion.Methods A retrospective analysis was performed on 93 older patients with ischemic stroke due to anterior circulation large vessel occlusion hospitalized in the First People′s Hospital of Zhengzhou City from January 2018 to March 2021.According to modified Rankin Scale(mRS)score,the patients were divided into a good prognosis group(mRS≤2)and a poor prognosis group(mRS>2).The clinical data were collected.Results Univariate analysis showed that the poor prognosis group had higher white blood cell count,neutrophil ratio,NLR,admission National Institute of Health Stroke Scale(NIHSS)score,and lower red blood cell count compared with the good prognosis group,the differences were statistically significant(P<0.05).Multivariate logistic regression analysis indicated that admission NIHSS score and NLR were independent risk factors for short-term poor prognosis in patients with ischemic stroke due to anterior circulation large vessel occlusion(P<0.05).Receiver operating characteristic curve analysis showed that the area under the curve of NLR to predict the short-term poor prognosis of stroke was 0.852(95%CI:0.778-0.926,P<0.001).Conclusion NLR has predictive value for the short-term prognosis in patients with ischemic stroke due to anterior circulation large vessel occlusion.
