摘　　要：目的:通过高通量体外药敏实验(HDS)评估晚期胃癌对不同治疗药物的抑制率,筛选其敏感的药物,制定、优化患者"个体化"治疗方案。方法:获取16例晚期胃癌患者的胃癌细胞,通过HDS技术检测原代胃癌细胞中所有可能对胃癌有效的抗癌药物的单药浓度梯度及其组合的高通量体外药敏测试,并根据药敏结果指导临床用药,进行疗效评估。结果:大部分原代胃癌细胞对化疗药物呈泛耐药性,尤其是含奥沙利铂在内的化疗方案在体外药敏试验中表现出较差的肿瘤抑制率。相反,硼替佐米表现出较高的敏感性。紫杉类药物/方案显示出略高于铂类的肿瘤抑制率。16例患者客观缓解率(ORR)为37.5%,疾病控制率(DCR)为75%。结论:晚期胃癌细胞对化疗药物呈现广泛耐药性,HDS检测在胃癌化疗药物的筛选需要进一步探索。Objective To evaluate the inhibition ratio of advanced gastric cancer to different therapeutic drugs by high-throughput in vitro drug sensitivity test,screen their sensitive drugs,and finally develop and optimize the patient’s"individualized"treatment plan.Methods Gastric cancer cells were obtained from 16 patients with advanced gastric cancer,and the single-agent concentration gradient and its combination of all anticancer drugs that may be effective for gastric cancer could be covered in primary gastric cancer cells by high-throughput drug sensitivity(HDS)technology for high-throughput in vitro drug susceptibility testing,and clinical medication was guided according to drug susceptibility results for efficacy evaluation.Results Most primary gastric cancer cells showed pan-resistance to chemotherapeutic agents,especially chemotherapy regimens containing oxaliplatin showed poor tumor inhibition rates in vitro susceptibility testing.In contrast,bortezomib showed high sensitivity.Taxanes/regimens showed slightly higher tumor inhibition rates than platinum.Sixteen patients had an objective response rate(ORR)of 37.5%and a disease control rate(DCR)of 75%.Conclusion Advanced gastric cancer cells present extensive resistance to chemotherapeutic drugs,HDS detection in the screening of chemotherapeutic drugs for gastric cancer needs further exploration.
