严重眼外伤折叠式人工玻璃体球囊植入的早期效果观察

摘　　要：目的观察严重眼外伤应用折叠式人工玻璃体球囊(FCVB)的临床效果。方法回顾性病例系列研究。分析唐山市眼科医院2019年6月至2022年6月严重眼外伤行折叠式人工玻璃体球囊植入术20例(20眼)的临床资料。其中7例为严重眼球切裂伤, 行Ⅰ期FCVB植入, 13例为外伤术后硅油依赖眼, 行FCVBⅡ期植入。所有患者术后至少随访3个月, 平均(5.8±1.6)个月。观察术后眼外观、眼压、视力、眼前节、眼轴、视网膜复位情况、球囊情况、术后反应、并发症及患者满意度。结果至末次随访, 术后视力提高4例, 视力无明显变化16例;术后眼压(8.95±2.28)mmHg(1 mmHg=0.133 kPa);视网膜均成功复位, 贴附良好;术后未出现球囊破损、球囊排斥、继发性青光眼、眼内炎或严重角膜变性等并发症。眼球外观良好, 未出现明显眼球萎缩, 眼轴(23.27±1.19)mm, 对外观满意者17例, 基本满意者3例。结论折叠式人工玻璃体球囊治疗严重眼外伤可支撑视网膜, 维持正常眼球形态且早期随访无严重并发症。Objective To observe the clinical efficacy of foldable capsular vitreous body(FCVB)in the treatment for severe ocular trauma.Methods This was a retrospective case series study.The clinical data of 20 eyes of 20 cases with severe ocular trauma who received FCVB implantation from Jun.2019 to Jun.2022 in Tangshan Eye Hospital were analyzed.Seven cases with severe ruptured eyes received FCVB implantion in phaseⅠ,and 13 cases with silicone oil dependent eyes after trauma received FCVB implantion in phaseⅡ.All patients were followed up for at least 3 months after surgery,with an average of(5.8±1.6)months.The appearance of the eyes,IOP,visual acuity,anterior segment,eye axis,retinal reduction,balloon condition,postoperative response,complications and patients’satisfaction after surgery were observed.Results At the last follow-up,the postoperative visual acuity improved in 4 cases,and resulted with no significant changes in 16 cases.The postoperative intraocular pressure was(8.95±2.28)mmHg(1 mmHg=0.133 kPa).The retina was successfully restored and well attached,there were no postoperative complications such as the foldable capsular vitreous body damage and rejection,secondary glaucoma,endophthalmitis,or severe corneal degeneration.These eyeballs had good appearances.Eyeball atrophy was not detected.And the average eye axis was(23.27±1.19)mm.There were 17 cases with satisfied appearance,3 cases with basic satisfaction.Conclusion FCVB in the treatment of severe eye trauma can support the retina,maintain normal eye morphology,and without serious complication at early-stage follow-up.
