摘 要:目的观察Ⅱ期人工晶状体悬吊植入术中应用微小角膜切口无黏弹剂技术的安全性及有效性。方法前瞻性病例研究。将2017年1月至2019年11月解放军989医院收治的无晶状体眼60例(60眼)纳入研究,所有患者均行无晶状体眼Ⅱ期微小(2.2 mm)角膜切口无黏弹剂应用的人工晶状体睫状沟悬吊手术。术后随访12~36个月,分析患者术后手术源性散光、术后角膜水肿情况、比较术前及术后眼压、角膜内皮细胞密度、超声生物显微镜检查结果及并发症。结果术后1 d,角膜透明;眼压为(19.60±1.50)mmHg,与术前眼压差异无统计学意义(t=0.37,P=0.726);术后1周角膜内皮细胞密度为(1986.36±101.18)个/mm^(2),与术前差异无统计学意义(t=0.22,P=0.856)。术后无人工晶状体脱位或移位,无缝线裸露或排斥反应发生。无眼内出血、视网膜脱离或脉络膜脱离等并发症发生。结论人工晶状体Ⅱ期悬吊术中微切口无黏弹剂技术并发症少,术后眼压稳定,对角膜内皮的保护作用良好,具有有效性和安全性。Objective To obverse the efficacy and safety of application of corneal microincision combined with non-viscoelastic technique during secondary suspension implantation of IOL.Methods This was a prospective case study.Total of 60 aphakia of 60 patients who underwent secondary intraocular lens suspension implantation in PLA No.989 Hospital were included.All patients underwent stageⅡintraocular lens implantation with 2.2 mm micro corneal incision and non viscoelastic intraocular lens ciliary groove suspension.The follow-up period was from 12 to 36 months.The postoperative astigmatism,intraocular pressure,corneal edema,corneal endothelial cell density,ultrasound biomicroscopy,and complications were analyzed.Results At 1 day after operation,the cornea was transparent.The intraocular pressure was(19.6±1.5)mmHg and had no significant difference from that before operation(t=0.37,P=0.726).The corneal endothelial cell density at 1 week after operation was(1986.36±101.18)cells/mm^(2),which had no significant difference from that before operation(t=0.22,P=0.856).There was no dislocation,displacement,suture exposure or rejection of the intraocular lens.There were no complications such as intraocular hemorrhage,retinal detachment or choroidal detachment.Conclusion Microincision combined with non-viscoelastic technique during secondary suspension of intraocular lens has less complications,stable intraocular pressure,good protection of corneal endothelium,and good safety and effectiveness.
