摘　　要：目的通过调查宜宾市七县三区公立医院的癫痫诊治水平,为今后开展对应的培训提供思路,为政府部门提供决策依据。方法对宜宾市13家公立医院的科室主任、一般医生开展调查问卷,内容涉及医院级别、神经内科医生进修癫痫情况、癫痫临床路径管理、抗癫痫药物构成、癫痫诊治设备构成等,并进行汇总分析。结果在13家医院中,分布最广的抗癫痫药物是丙戊酸和卡马西平,而新型抗癫痫药奥卡西平、拉莫三嗪、托吡酯的分布均没有超过50%;癫痫的主要监测设备——普通脑电图在53.8%的医院中存在,而更加强大的视频脑电图仅15.4%的医院具备。在13家医院中,有2家医院各有2名医生曾参加癫痫方向进修,4家医院各有1名医生曾参加癫痫方向进修,其余7家医院(53.8%)均没有医生参加癫痫方向进修。在癫痫临床路径管理中,发现仅有5家医院开展了癫痫路径管理,其余8家医院(61.5%)均未开展。结论癫痫的诊治水平在宜宾市范围内的各级医院中仍较薄弱,硬件设备及人员培训、管理制度等还有待进一步加强。Objective To investigate the diagnosis and treatment level of epilepsy in public hospitals of Yibin City,for providing ideas for corresponding training in the future,and provide decision-making basis for government departments.Methods The questionnaire survey was conducted among doctors working in 13 public hospitals of Yibin city,that covering the hospital level,status of neurology physicians undergoing epilepsy training,clinical pathway management of epilepsy,composition of antiepileptic drugs,and composition of equipment for diagnosis and treatment of epilepsy.Results In 13 hospitals,valproic acid and carbamazepine were the most widely distributed antiepileptic drugs,while the distribution of Oxazepine,lamotrigine and topiramate was not more than 50%.EEG was present in 53.8 percent of hospitals,while the video EEG was available in only 15.4 percent.Among the 13 hospitals,2 doctors who had participated in the epilepsy orientation training worked in 2 hospitals,1 doctor who had participated in the epilepsy orientation training worked in 4 hospitals,and the other 7 hospitals(53.8%)had no doctors who had participated in the epilepsy orientation training.There were only 5 hospitals which carried out epilepsy pathway management.The remaining eight hospitals(61.5%)did not operate.Conclusion The level of diagnosis and treatment of epilepsy was still weak in hospitals of Yibin city,and the equipment,personnel training and management system should be further strengthened.
