Protaper与Orodeka根管预备方案对根管治疗期间急症和充填效果的影响  

Influence of Protaper and Orodeka Root Canal Preparation Protocols on Interappointment Emergencies and Filling Effects

摘　　要：目的:比较Protaper根管锉与Orodeka根管锉根管预备方案分别对根管治疗期间急症(IAE)和充填效果的影响。方法:选取2019年2月-2022年2月常州市第四人民医院因牙髓炎或根尖周炎需要做根管治疗的患者126例,恒磨牙126颗(392个根管),随机分为A组63例和B组63例,其中A组根管193个,B组根管199个。A组使用Protaper机用镍钛锉根管预备方案,B组使用Orodeka镍钛锉根管预备方案。预备完成后根管内封氢氧化钙糊剂,复诊时以侧压法进行常规根管充填,比较两组IAE和充填效果。结果:术后即刻及术后7 d两组IAE发生情况比较差异无统计学意义(P>0.05),术后1、3 d,B组IAE发生率低于A组,B组总体情况优于A组(P<0.05);两组根管预备后弯曲度变化差异有统计学意义(P<0.05);两组预备后填充时间、长度适充率、三维严密度、充填成功率比较差异无统计学意义(P>0.05)。结论:Protaper与Orodeka根管预备方案在根管充填方面均能达到良好效果,无显著差异。但Orodeka根管预备方案相比于Protaper方案可降低IAE发生情况,降低患者疼痛反应,并且对根管弯曲度影响更小。Objective:To compare the influence of root canal preparation schemes based on Protaper and Orodeka files on the effects of interappointment emergencies(IAE)and filling during root canal treatment,respectively.Method:A total of 126 patients who needed root canal therapy due to pulpitis or periapical periodontitis in Changzhou Fourth People’s Hospital from February 2019 to February 2022 were selected.There were 126 permanent molars(392 root canals).They were randomly divided into 63 cases of group A and 63 cases of group B,including 193 root canals in group A and 199 root canals in group B.Group A used the Protaper machine with nickel-titanium file for root canal preparation,and group B used the Orodeka nickel-titanium file for root canal preparation.After the preparation,the root canal was sealed with calcium hydroxide paste,and routine root canal filling was performed by lateral condensation method during the follow-up visit.The IAE and filling effects were compared between the two groups.Result:There was no significant difference in IAE occurrence between the two groups immediately after surgery and 7 d after surgery(P>0.05).The incidence of IAE in group B was lower than that in group A on 1 and 3 d after surgery,and the overall situation in group B was better than that in group A(P<0.05).There was significant difference in the changes of curvature after root canal preparation between the two groups(P<0.05).There were no significant differences in the change of filling time,filling length,three-dimensional tightness,and filling success rate after treatment between the two groups(P>0.05).Conclusion:Both Protaper and Orodeka root canal preparation schemes can achieve good results in root canal filling,and there is no significant difference.However,the Orodeka root canal preparation scheme can reduce the occurrence of IAE and reduce the pain response of patients compared with the Protaper scheme,and less impact on root canal curvature.
