摘 要:目的探讨Toll样受体4(tol-likereceptor4,TLR4)信号通路下游的髓样分化因子88(myeloid differentiation factor88,MyD88)和β干扰素Toll/IL-1受体结构域衔接蛋白(toll-like receptor-associated activator of interferon,TRIF)在皮肤恶性黑素瘤(cutaneous malignant melano ma,CMM)中的表达及与临床特征之间的关联。方法应用免疫组化方法检测40例CMM患者肿瘤组织、10例痣细胞痣患者皮损组织和10例健康皮肤组织中MyD88和TRIF的表达水平,采用SPSS22.0统计学软件进行t检验及Pearson检验处理分析。结果CMM中的MyD88及TRIF的表达强度显著高于痣细胞痣与正常皮肤组织(P<0.05);MyD88、TRIF的表达与CMM患者的年龄、性别等因素无明显相关性,而与发病部位、Clark分级、Breslow厚度、溃疡等因素相关,其中发生于肢端相较于非肢端的恶性黑素瘤表达更显著(P-0.002,0.001);随着Clark分级的增加,原位与浸润肿瘤之间,MyD88、TRIF的表达水平也增加(P=0.001,0.012);随着Breslow厚度的增加,两者表达水平也增加(P=0.004,0.026);合并溃疡的CMM中,MyD88表达增加,有统计学意义(P=0.019);TRIF表达强度无统计学差异(P>0.05);MyD88和TRIF均与CMM患者的淋巴结转移无相关性(P0.05)。结论TLR4下游的MyD88和TRIF相互协同,共同参与CMM的发生与发展。ObjectiveTo investigate the expression of MyD88(Myeloid Differentiation Factor 88)and TRIF(toll-like receptor-associated activator of interferon)of the TLR4(toll-like receptor 4)signaling pathway in cutaneous malignant melanoma and the association between clinical features in cutaneous malignant melanoma.Methods The immunohistochemical method was used to detect the expression of MyD88 and TRIF in 40 cases of cutaneous malignant melanoma,10 cases of sputum cell sputum and 10 cases of healthy skin tissue.The specific data of this experiment were analyzed by SPSS22.0 statistical software.ResultsThe intensity of MyD88 and TRIF in cutaneous malignant melanoma were significantly higher than sputum cell sputum and normal skin tissue(P<0.05).The expression of MyD88 and TRIF was not related to age and gender,but related to the site of the tumor,Clark level,Breslow thickness,tumor ulceration,etc.Among them,the expression of MyD88 and TRIF in cutaneous malignant melanoma was significantly higher inthe limb than the non-limb(P=0.002,0.001).With the increased of Clark lever,the expression of MyD88 and TRIF also statistically increased(P-0.001,0.012).With the increased of Breslow thickness,the expression of MyD88 and TRIF was also statistically increased(P-0.004,0.026).The expression of MYD88 statistically increased in the melanoma with ulceration(P=0.019),however the expression of TRIF did not have a statistical difference(P>0.05).The expression of MYD88 and TRIF were uncorrelated to lymph node metastasis(P>0.05).ConclusionMyD88 and TRIF of the TLR4 signaling pathway synergize with each other toparticipat in the occurrence and development of cutaneous malignant melanoma.
