摘 要:目的:使用CBCT测量牙周炎患者的上颌窦黏膜厚度,探讨施耐德膜增厚与相应上颌磨牙牙周状态的关系。方法:收集247名牙周炎患者的CBCT图像,测量并记录年龄、性别、骨吸收最严重的上颌磨牙牙位、相应磨牙的牙周骨丧失(periodontal bone loss, PBL)、相应磨牙的骨吸收方式、施耐德膜最大黏膜厚度(MMT)、最小剩余牙槽骨厚度(residual alveolar bone thickness, minRBT)。施耐德膜厚度>2 mm为病理性增厚,分析施耐德膜厚度的影响因素。使用二元logistic回归分析研究其相关性并构建森林图。结果:121例患者(49.0%)CBCT显示施耐德膜增厚,黏膜增厚与年龄、性别、minRBT、骨吸收方式和牙周骨丧失有相关性(P<0.05)。男性相对于女性发生施耐德膜增厚的风险增加(OR=2.1,95%CI 1.19~3.62,P=0.01)。minRBT≤2 mm较minRBT≥10 mm发生施耐德膜增厚的风险增加(OR=10.2,95%CI 2.40~43.92,P<0.05)。结论:性别和minRBT是牙周炎患者施耐德膜增厚的危险因素,提示及时进行牙周治疗以改善minRBT,降低施耐德膜增厚的可能性。Objective:The thickness of maxillary sinus mucosa in patients with periodontitis was measured by CBCT to investigate the relationship between Schneider membrane thickening and periodontal status of maxillary molars.Methods:CBCT images of 247 patients with periodontitis were collected.Age, gender, maxillary molar position with the most severe bone resorption, corresponding periodontal bone loss(PBL),bone resorption pattern, the maximum mucosal thickness of Schneider membrane(MMT),and minimum residual alveolar bone thickness(minRBT)were measured and recorded.Schneider film thickness greater than 2 mm was considered pathological thickening.The influencing factors of Schneider film thickness were analyzed.Chi-square test and binary logistic regression analysis were used to study the correlation and construct a forest map.Results:Totally 121 CBCT images(49.0%)showed thickening of the Schneider membrane.Mucosal thickening was correlated with age, gender, minRBT,bone absorption mode and alveolar bone loss(P<0.05).There was an increased risk of Schneider membrane thickening in males compared with females(OR=2.1,95%CI 1.19~3.62,P=0.01).The risk of Schneider film thickening was increased with minRBT≤2 mm compared with minRBT≥10 mm(OR=10.2,95%CI 2.40~43.92,P<0.05).Conclusion:Gender and minRBT are risk factors for Schneider membrane thickening in patients with periodontitis, suggesting timely periodontal treatment to improve minRBT and reduce the possibility of Schneider membrane thickening.
