摘 要:目的 分析结直肠癌组织中连接蛋白43(connexin43, CX43)的表达与临床病理特征的关系,以及对短期疗效与长期预后的影响。方法 选取本院2015年1月~2017年4月期间收治的58例接受奥沙利铂联合5-FU衍生物化疗的晚期结直肠癌患者为研究对象,免疫组织化学染色法分析患者结直肠癌组织中CX43蛋白的表达情况。探讨CX43蛋白表达水平与临床病理特征、化疗疗效及生存期之间的关系。结果 58例患者癌组织中,CX43蛋白阳性表达33例,阴性表达25例。CX43表达阳性的患者其组织分化程度为高、中分化的比率明显高于CX43表达阴性患者(P<0.05)。在CX43表达阳性的结直肠癌患者中,Dukes分期为A、B期的比率显著高于CX43表达阴性的患者,Dukes分期为C+D期的比率显著低于CX43表达阴性的患者(P<0.05)。CX43表达阳性的结直肠癌患者存在淋巴结转移阳性的比率明显低于CX43表达阴性的患者,Ki67高表达的比率显著高于CX43表达阴性的患者(P<0.05)。CX43阳性表达组患者的化疗有效率、中位PFS和OS均显著高于CX43阴性表达组(P<0.05)。结论 CX43蛋白表达与结直肠癌分化程度、Dukes分期、淋巴结转移、短期疗效、长期预后等均有密切关系。CX43蛋白表达异常可能是结直肠癌发生发展过程中的重要影响因素。Objective To analyze the relationship between CX43 protein expression and clinicopathological features in colorectal cancer tissues, as well as its effect on short-term efficacy and long-term prognosis.Methods Fifty-eight patients with advanced colorectal cancer who received oxaliplatin combined with card-5-FU derivative chemotherapy in our hospital from January 2015 to April 2017 were selected as the research objects, and the expression of CX43 protein in colorectal cancer tissues of the patients was analyzed by immunohistochemical staining. To investigate the relationship between CX43 protein expression level and clinicopathological features, chemotherapy efficacy and survival. Results CX43 protein was positive in 33 cases and negative in 25 cases among 58 patients. The proportion of patients with high or medium differentiation in CX43 positive colorectal cancer was significantly higher than that in CX43 negative colorectal cancer patients(P<0.05).Among patients with CX43 positive colorectal cancer, the proportion of patients with Dukes stage A and B was significantly higher than that of patients with CX43 negative expression.The proportion of patients with Dukes stage C+D was significantly lower than that of patients with negative CX43 expression(P<0.05).Among patients with colorectal cancer with positive CX43 expression, the percentage of patients with positive lymph node metastasis was significantly lower than that of patients with negative CX43 expression(P<0.05). The proportion of patients with high Ki67 expression was significantly higher than that with negative CX43 expression(P<0.05). The chemotherapy response rate, median PFS and OS in CX43 positive group were significantly higher than those in CX43 negative group(P<0.05). Conclusion CX43 protein expression is closely related to colorectal cancer differentiation, Dukes stage, lymph node metastasis, short-term efficacy, and long-term prognosis.
