摘 要:目的探讨不同多巴胺D2受体(DRD2)TaqI A基因型海洛因依赖者伏隔核的功能磁共振成像(fMRI)连接。方法招募31例A1+型与25例A1-型海洛因依赖者,收集人口学特征,测量渴求、厌恶、焦虑和抑郁心理量表,采集静息态fMRI数据。以双侧伏隔核为种子点进行功能连接分析。结果2组间在人口学特征与心理学评分均无显著性差异(P>0.05)。与A1-型海洛因依赖者相比,A1+型双侧伏隔核均与双侧眶额回功能连接增强[P<0.01,错误发现率(FDR)校正],与双侧内侧前额叶和楔前叶功能连接降低(P<0.01,FDR校正)。A1+型海洛因依赖者左侧伏隔核与眶额回的功能连接强度与渴求评分呈正相关(r=0.658,P<0.001)。结论A1+型海洛因依赖者驱动反应对奖赏刺激可能更具有敏感性,执行控制功能损伤更严重。Objective To investigate the functional connectivity of the nucleus accumbens in heroin addicts with different dopamine D2 receptor(DRD2)TaqI A genotypes via functional magnetic resonance imaging(fMRI).Methods Thirty-one A1+and 25 A1-heroin addicts were recruited.Demographic characteristics were collected and psychological scales such as craving,aversion,anxiety and depression were measured.The resting-state fMRI data of all subjects were collected.Functional connectivity was performed via considering bilateral nucleus accumbens as seeds.Results There were no significant differences in demographic characteristics and psychological scores between the two groups(P>0.05).Compared with the A1-heroin addicts,the functional connectivity of A1+heroin addicts between the bilateral nucleus accumbens and the bilateral orbitofrontal gyrus were increased[P<0.01,false discovery rate(FDR)corrected],while the functional connectivity with the bilateral medial prefrontal and precuneus lobes were decreased in the A1+heroin addicts(P<0.01,FDR corrected).The strength of functional connectivity between left nucleus accumbens and orbitofrontal gyrus was positively correlated with craving score in A1+heroin addicts(r=0.658,P<0.001).Conclusion A1+heroin addicts may be more sensitive to reward stimuli and have deficits in executive control.
