摘  要:目的初步探讨脑血流灌注改变对T_(2)液体衰减反转恢复(T_(2)FLAIR)-扩散加权成像(DWI)不匹配及T_(2)mapping值改变评估缺血性脑卒中发作时间准确性的影响。方法对发作时间<24 h患者进行3.0T MR扫描。扫描序列包括DWI、Synthetic MRI、T_(2)FLAIR、三维伪连续动脉自旋标记(3D pcASL)。对扫描获取的脑血流量(CBF)map、T_(2)map和T_(2)FLAIR图像分别测量并计算DWI受限区域与对侧正常区域的信号值、信号差值和信号比值。将患者分为CBF正常组(CBF>25 mL·100 g^(-1)·min^(-1))和CBF减低组(CBF≤25 mL·100 g^(-1)·min^(-1))。所有数据采用SPSS 25.0软件进行统计学分析。结果在CBF减低组中,脑卒中发病时间与脑扩散受限区域的T_(2)值及T_(2)FLAIR信号改变存在相关性,其中T_(2)差值最明显(r=0.810,P=0.000),T_(2)比值(r=0.793,P=0.000)和T_(2)值(r=0.722,P=0.002)次之,相关性最弱的是T_(2)FLAIR比值(r=0.571,P=0.021)。而在CBF正常组中,脑卒中发病时间与T_(2)值及T_(2)FLAIR信号改变无显著相关。结论缺血性脑卒中患者,脑卒中发病时间与T_(2)FLAIR信号改变及T_(2)值改变的相关性都受到脑血流灌注状态的影响,其中T_(2)值改变与缺血性脑卒中发病时间相关性高于T_(2)FLAIR信号改变。Objective To investigate the effect of cerebral blood flow perfusion changes on the accuracy of T_(2)fluid attenuated inversion recovery(T_(2)FLAIR)-diffusion weighted imaging(DWI)mismatch and T_(2)mapping changes in the assessment of onset time of ischemic stroke.Methods Patients with onset time less than 24 h underwent 3.0T MR scan.The scanning sequences included DWI,Synthetic MRI,T_(2)FLAIR and three-dimensional pseudo-continuous arterial spin labeling(3D pcASL).In the cerebral blood flow(CBF)map,T_(2)map and T_(2)FLAIR images,the signal intensities,signal difference and signal ratio of the DWI restricted region and the contralateral normal region were measured and calculated,respectively.Patients were divided into normal CBF group(CBF>25 mL·100g^(-1)·min^(-1))and reduced CBF group(CBF≤25 mL·100g^(-1)·min^(-1)).All data were statistically analyzed by SPSS 25.0 software.Results In the reduced CBF group,the onset time of stroke was correlated with T_(2)value and T_(2)FLAIR signal changes in the brain diffusion restricted region,and T_(2)difference was the most obvious(r=0.810,P=0.000),T_(2)ratio(r=0.793,P=0.000)and T_(2)value(r=0.722,P=0.002)followed,and the weakest correlation was T_(2)FLAIR ratio(r=0.571,P=0.021).In the normal CBF group,there was no significant correlation between stroke onset time and T_(2)value and T_(2)FLAIR signal changes.Conclusion In patients with ischemic stroke,the correlation between stroke onset time and T_(2)FLAIR signal changes and T_(2)value changes are affected by cerebral blood perfusion status,and the correlation between T_(2)value changes and ischemic stroke onsite time is higher than that of T_(2)FLAIR signal changes.
