膀胱非上皮性良性肿瘤CT及MRI表现并文献回顾

摘　　要：目的探讨膀胱非上皮性良性肿瘤影像学表现。方法回顾性分析14例膀胱非上皮性良性肿瘤的CT及MRI影像资料,并与病理结果相对照。结果平滑肌瘤5例,CT平扫呈等密度,增强扫描动脉期轻度强化,静脉期、延迟期持续强化;T1WI呈等或低信号,T2WI呈等或低信号,增强扫描动脉期轻中度或明显强化,静脉期及延迟期渐进性、持续强化。副神经节瘤5例,CT平扫呈等密度,增强扫描动脉期明显强化,静脉期、延迟期强化程度减低;T1WI呈等、高混杂信号,T2WI呈高、低混杂信号,增强扫描动脉期明显欠均匀强化,静脉期及延迟期强化稍减退。炎性肌纤维母细胞瘤2例,CT平扫呈等密度,增强扫描动脉期及静脉期明显均匀强化,延迟期强化明显减低;T1WI呈等、稍低信号,T2WI呈等、稍高信号,增强扫描动脉期明显不均匀强化,静脉期及延迟期渐进性强化。脉管瘤2例,CT平扫呈等、低混杂密度,增强扫描动脉期轻度不均匀强化,静脉期及延迟期渐进性强化;T1WI及T2WI呈混杂信号,增强扫描动脉期不均匀轻度强化,静脉期及延迟期渐进性强化。结论膀胱非上皮性良性肿瘤一般表现为单发,圆形或类圆形、边界光整,动态增强扫描常表现为持续性、渐进性强化,无淋巴结转移征象,对术前明确诊断及鉴别诊断具有提示作用,最终确诊仍需依靠病理学检查。Objective To investigate the imaging features of non-epithelial benign tumors of bladder.Methods CT and MRI imaging data of 14 cases of bladder non-epithelial benign tumors were retrospectively analyzed and compared with pathological results.Results There were 5 cases of leiomyomas,plain CT showed isodensity,enhanced CT showed mild enhancement in arterial phase,continuous enhancement in venous and delayed phase;T1WI showed equal or low signal,T2WI showed equal or low signal,mild-to-moderate or significant enhancement in arterial phase,gradual and continuous enhancement in venous and delayed phase.There were 5 cases of paragangliomas,plain CT showed isodensity,enhanced significantly in arterial phase,but decreased in venous and delayed phase;T1WI showed inhomogeneously equal and high signal,T2WI showed inhomogeneously high and low signal,the enhancement of arterial phase was obviously uneven,and the enhancement of venous and delayed phase was slightly decrease.There were 2 cases of inflammatory myofibroblastomas,plain CT showed isodensity,enhanced arterial and venous phase significantly uniform enhancement,delayed phase enhancement significantly decreased;T1WI showed equal and slightly low signal,T2WI showed equal and slightly high signal,enhanced obviously uneven in arterial phase,gradual enhancement in venous and delayed phase.There were 2 cases of angiomas,plain CT showed inhomogeneously equal and low density,mild uneven enhancement in arterial phase on enhanced CT,gradual enhancement in venous and delayed phase on enhanced CT;inhomogeneous signal on T1WI and T2WI,mild uneven enhancement in arterial phase,gradual enhancement in venous and delayed phase on enhanced.Conclusion Non-epithelial benign tumors of bladder are usually single,round or quasi-round,with smooth boundary.Dynamic enhanced scan usually shows continuous and gradual enhancement,without lymph node metastasis.It is suggestive for preoperative diagnosis and differential diagnosis,and the final diagnosis still depends on pathological examination.
