摘　　要：目的探讨HEOS宫腔镜冷刀治疗育龄期子宫黏膜下肌瘤患者的有效性。方法选取2017年1月—2020年12月在江苏大学附属宜兴医院妇科住院的50例子宫黏膜下肌瘤患者为研究对象,按照随机数表法分为观察组25例、对照组25例。观察组采用HEOS宫腔镜冷刀切除,对照组采用传统宫腔镜电切。比较两组患者术后月经改善率,肌瘤切净率、肌瘤复发率及宫腔粘连发生率。结果观察组术后月经改善率92.00%、肌瘤切净率96.00%均高于对照组,差异有统计学意义(χ^(2)=4.500、4.878,P<0.05);观察组术后肌瘤复发率、宫腔黏连发生率均低于对照组,差异有统计学意义(χ^(2)=8.000、4.735,P<0.05)。结论HEOS宫腔镜冷刀切除子宫黏膜下肌瘤相比于传统的电切治疗更为安全、有效,可以保护子宫内膜,在一定程度上提高受孕概率,可作为治疗育龄期子宫黏膜下肌瘤新的方法推广应用。Objective To investigate the effectiveness of HEOS hysteroscopic cold knife treatment in patients with submucosal fibroids in the reproductive age.Methods Fifty patients with submucosal fibroids who were hospitalized in the Department of Gynecology and Obstetrics,Yixing Hospital,Jiangsu University from January 2017 to December 2020 were selected for the study.The patients were divided into 25 cases in the observation group and 25 cases in the control group according to the random number table method.The observation group was resected by HEOS hysteroscopic cold knife,and the control group was resected by traditional hysteroscopic electrodes.The postoperative menstrual improvement rate,myomectomy rate,myoma recurrence rate and uterine adhesion incidence rate were compared between the two groups.Results The postoperative menstrual improvement rate 92.00%and myoma clearance rate 96.00%were higher in the observation group than in the control group,and the differences were statistically significant(χ^(2)=4.500,4.878,P<0.05);yhe postoperative myoma recurrence rate and the incidence of uterine adhesions in the observation group were lower than those in the control group,and the differences were statistically significant(χ^(2)=8.000,4.735,P<0.05).Conclusion HEOS hysteroscopic cold knife resection of submucosal fibroids is safer and more effective than traditional electrodesiccation treatment,which can protect the endometrium and improve the probability of conception to a certain extent,and can be promoted as a new method for the treatment of submucosal fibroids in the fertile age.
