摘 要:目的探讨基底动脉瘤的血液流变特性。方法以基底动脉MRA图像为基础构建基底动脉三维几何模型,分为动脉瘤组与正常组别,应用计算流体力学方法对基底动脉进行数值模拟分析。结果获得个体化基底动脉瘤模型的血流动力学参数,数值模拟结果包括基底动脉的动脉瘤发生指数(aneurysm formation indicator,AFI)、梯度震荡数值(gradient oscillatory number,GON)及震荡剪切系数(oscillatory shear stress,OSI);基底动脉瘤组动脉瘤处及右侧椎动脉具有低AFI、高GON及高OSI值。结论基底动脉瘤形成可能与低AFI、高GON及高OSI值有关,动脉瘤内血流方向变化较大及不稳定血流,血管壁损伤概率增加,动脉瘤破裂风险较大,临床上需及早服用药物进行治疗或进行外科手术干预。Objective To investigate the hemorheological characteristics of basilar artery aneurysm.Methods The three-dimensional geometric model of basilar artery was constructed based on MRA images of basilar artery and divided into normal groups of aneurysms.The basilar artery was numerically simulated and analyzed by computational fluid dynamics.Results The hemodynamic parameters of the individualized basilar aneurysm model were obtained,and the numerical simulation results included the aneurysm formation index(AFI),gradient oscillatory number(GON),and oscillatory shear stress(OSI)of the basilar artery;The aneurysm site and the right vertebral artery in the basilar aneurysm group had low AFI,high GON,and high OSI values.Conclusion Basilar artery aneurysm formation may be related to low AFI,high GON and high OSI values,large changes in the direction of blood flow in the aneurysm and unstable blood flow,increased probability of vessel wall injury,and greater risk of aneurysm rupture,which requires early medication or surgical intervention in clinical practice.
