摘  要:目的通过生物电阻抗分析法测得肺移植患者术前的身体成分,探究其与术后临床结局间的关系。方法收集2020年9月至2021年6月入住无锡市人民医院呼吸与危重症科病房的进行术前检查的终末期肺病患者的身体测量指标、生化检查指标和人体成分结果。患者手术后随访并收集其临床结局资料,包括:机械通气时间、术后1个月肺部感染情况、术后3个月死亡情况。结果相位角是术后1个月肺部感染发生的独立保护因素(OR=0.077,95%CI 0.006~0.983),是术后3个月受者死亡发生的独立保护因素(OR=0.122,95%CI 0.017~0.860),是机械通气时间的显著影响因素(P<0.05)。结论相位角是肺移植受者术前营养状况重要的评价参数,低相位角与机械通气时间延长有关,也是移植术后1个月肺部感染和术后3个月死亡的危险因素。Objective To explore the relationship between body composition indices and postoperative clinical outcomes after lung transplantation using bioelectrical impedance analysis to analyze body compositions of lung transplant patients before surgery.Methods The clinical data of body measurement indices,biochemical indicators and body composition indices were collected for end-stage lung disease patients,who were admitted to the respiratory and critical care ward of Wuxi People’s Hospital from September 2020 to June 2021 for preoperative examinations.The patients after surgery were followed up and collected with their clinical outcome data,including:duration of mechanical ventilation,pulmonary infection in one month after surgery,and death in three months after surgery.Results Phase angle was an independent protective factor for the occurrence of lung infection in the first month after surgery(OR=0.077,95%CI 0.006-0.983),an independent protective factor for death of recipients in three months after surgery(OR=0.122,95%CI 0.017-0.860),and significantly influenced the duration of mechanical ventilation(P<0.05).Conclusion Phase angle is an important parameter for evaluating the nutrition status of lung transplant recipients before surgery.Low phase angle is related to prolonged mechanical ventilation duration,and is also a risk factor for lung infection in one month after transplantation and death in three months after transplantation.
