中药子午流注热熨对妇科患者术后疼痛缓解及胃肠功能的影响

摘　　要：目的观察中药子午流注热熨对妇科患者术后疼痛缓解及胃肠功能的影响。方法选取2017年2月—2018年5月期间河北省保定市第一中心医院和保定市妇幼保健院收治的100例妇科术后患者,按照随机数字表法分为观察组和对照组,每组各50例。对照组在常规西医治疗和护理基础上于术后第1天配合普通热熨治疗,观察组在常规西医治疗和护理基础上于术后第1天加用中药子午流注热熨治疗。连续治疗4 d后,观察比较两组患者治疗后临床疗效、肠鸣音恢复时间、首次排气时间和排便时间、疼痛评分以及胃肠激素水平[血管活性肠肽(vasoactive intestinal peptide,VIP)、胃泌素(gastrin,GAS)、胃动素(motilin,MTL)]。结果治疗后观察组总有效率94.00%(47/50)明显高于对照组78.00%(39/50),差异有统计学意义(χ^(2)=5.316,P<0.05)。观察组术后肠鸣音恢复时间、首次排气和排便时间均明显低于对照组,差异有统计学意义(P<0.05)。对照组术后48 h、72 h疼痛评分均明显低于术后即时,差异有统计学意义(P<0.05),观察组术后24 h、48 h、72 h疼痛评分均明显低于术后即时,差异有统计学意义(P<0.05);且观察组术后24 h、48 h、72 h疼痛评分均明显低于对照组,差异有统计学意义(P<0.05)。两组患者治疗后VIP水平较治疗前均降低,GAS和MTL水平较治疗前均升高,差异有统计学意义(P<0.05),且观察组治疗后VIP、GAS、MTL改善程度明显优于对照组,差异有统计学意义(P<0.05)。结论中药子午流注热熨能够有效缓解妇科患者术后疼痛,调节胃肠激素水平,促进患者尽快恢复肠鸣音,缩短排气和排便时间,显著改善患者术后胃肠功能。Objective To observe the effect of Ziwu Liuzhu hot compress on pain relief and gastrointestinal function in patients with gynecological diseases after operation.Methods A total of 100 patients with gynecological diseases undergoing operationin the Department of Gynecology of Baoding Maternal and Child Health Hospital in Hebei Province from February 2017 to May 2018 were included and divided into an observation group and a control group by random number table,50 in each group.Patients in the control group were given ordinary hot compress on the first day after operation on the basis of conventional western medicine treatment and nursing,while those in the observation group were given Ziwu Liuzhu hot compress on the first day after operation on the basis of conventional western medicine treatment and nursing.After 4 consecutive days of treatment,the clinical efficacy,recovery time of bowel sounds,time of first anal exhaust and defecation,pain score and level of gastrointestinal hormones[vasoactive intestinal peptide(VIP),gastrin(GAS),motilin(MTL)]were compared between the two groups.Results The total effective rate of patients in the observation group was 94.00%(47/50),which was higher than 78.00%(39/50)in the control group(χ^(2)=5.316,P<0.05).The recovery time of bowel sounds and the time of first anal exhaust and defecation in the observation group were shorter than those in the control group(P<0.05).Compared with the immediate conditions after operation,the postoperative pain in the control group at 48 h and 72 h and that in the observation group at 24 h,48 h and 72 h were relieved(both P<0.05),and the pain scores in the observation group at 24 h,48 h and 72 h after operation were lower than those in the control group(P<0.05).After treatment,the VIP level of patients in both groups was decreased while the GAS and MTL levels were increased compared with the conditions before treatment(P<0.05),andthe improvement of VIP,GAS and MTL in the observation group after treatment was more significant than that in the cont
