保乳手术联合前哨淋巴结活检术对早期乳腺癌患者预后及乳房美容满意度的影响

摘　　要：目的探讨保乳手术联合前哨淋巴结活检术对早期乳腺癌患者预后、乳房美容满意度的影响。方法将102例早期乳腺癌患者根据手术方式的不同分为对照组(n=51)和观察组(n=51)。对照组患者采取改良根治术+腋窝淋巴结清扫术,观察组患者采用保乳手术+前哨淋巴结活检术。比较两组患者的手术相关指标、肩关节活动度、预后状况以及乳房美容满意度。结果观察组患者手术时间明显长于对照组,腋窝拔管时间、住院时间均明显短于对照组,术中出血量、总引流量均明显少于对照组,差异均有统计学意义(P﹤0.01)。术后6个月,两组患者肩关节外旋、内旋、后伸、外展、屈曲度均较术前减小,但观察组患者肩关节外旋、内旋、后伸、外展、屈曲度均大于对照组,差异均有统计学意义(P﹤0.05)。术后,观察组患者体象、性功能、性愉悦、对未来的看法评分均明显高于对照组,乳房症状、上肢症状评分均明显低于对照组,差异均有统计学意义(P﹤0.01)。观察组患者乳房美容总满意度高于对照组,差异有统计学意义(P﹤0.05)。结论早期乳腺癌患者行保乳手术联合前哨淋巴结活检术具有创伤小、缩短患者住院时间等优点,可以促进患者术后上肢功能恢复,提高患者生活质量,且患者乳房美容满意度较高。Objective To investigate the effect of breast-conserving surgery combined sentinel lymph node biopsy on the prognosis and satisfaction of breast beauty in patients with early breast cancer.Method A total of 102 patients with early breast cancer were divided into control group(n=51,treated with modified radical mastectomy+axillary lymph node dissection)and observation group(n=51,treated with breast-conserving surgery+sentinel lymph node biopsy)according to the operation method.The operation indicators,range of motion in shoulder joint,prognosis and satisfaction of breast beauty were compared between the two groups.Result The operation time of observation group was longer than that of control group,axillary extubation time,hospital stay,intraoperative blood loss and total drainage volume in the observation group were lower than those in the control group(P<0.01).At 6 months after operation,joint external rotation,internal rotation,extension,abduction,flexion degree in two groups were lower than those before operation,with higher range of motion in observation group than those in the control group(P<0.05).After operation,the scores of body image,sexual function,sexual pleasure and future view in the observation group were higher than those in the control group,the scores of breast symptoms and upper limb symptoms in the observation group were lower than those in the control group(P<0.01).The total satisfaction rate of breast beauty in the observation group was higher than that in the control group(P<0.05).Conclusion Breast conserving surgery+sentinel lymph node biopsy is less invasive,which can shorten the length of hospital stay,promote the recovery of upper limb function,and improve the quality of life and satisfaction of breast beauty in patients with early breast cancer.
