摘　　要：目的:探讨Th17/Treg细胞及相关细胞因子在重症肌无力(MG)发病中的作用。方法:收集2018年10月至2020年12月在广西医科大学第一附属医院门诊或住院确诊的28例MG患者(MG组)及23例健康对照(HC组)的外周血标本,采用流式细胞术检测两组外周血单个核细胞(PBMCs)中Th17细胞及Treg细胞比例;酶联免疫吸附试验(ELISA)检测血清中细胞因子(IL-17、IL-6及TGF-β)的表达水平;采用MG定量评分(QMG)评估患者病情严重程度,并分析Th17/Treg细胞比例与QMG评分之间的相关性。结果:与健康对照组比较,MG组外周血PBMCs中Treg细胞比例及Treg/Th17细胞比值升高(P<0.05),Th17细胞比例差异无统计学意义(P>0.05)。与健康对照组比较,MG组血清中Treg细胞相关的细胞因子TGF-β表达水平降低(P<0.05),IL-17、IL-6表达水平差异均无统计学意义(P>0.05)。Th17/Treg细胞比例与患者病情严重程度QMG评分无显著相关性(P>0.05)。结论:MG患者Th17/Treg细胞的转化及相关细胞因子水平存在一定的异质性。Objective:To investigate the role of Th17/Treg cells and associated cytokines in the pathogenesis of myasthenia gravis(MG).Methods:Peripheral blood samples were collected from 28 MG patients(MG group)and 23 healthy controls(HC group)who were diagnosed in the outpatient department or inpatient department of the First Affiliated Hospital of Guangxi Medical University from October 2018 to December 2020.Flow cytometry was used to detect the proportion of Th17 cells and Treg cells in peripheral blood mononuclear cells(PBMCs)of MG patients and healthy controls.The expressions of serum cytokines(IL-17,IL-6 and TGF-β)were measured by enzyme-linked immunosorbent assay(ELISA).Quantitative myasthenia gravis score(QMG)was used to evaluate the severity of the disease and the correlation between Th17/Treg cell ratio and QMG score was analyzed.Results:Compared with HC group,the proportion of Treg cells and the ratio of Treg/Th17 cells in PBMCs of MG group increased(P<0.05);there were no significant differences in the proportion of Th17 cells(P>0.05).Compared with HC group,the expression of Treg cell-related cytokine TGF-βin serum of MG group decreased(P<0.05).There were no significant differences in the expressions of IL-17 and IL-6(P>0.05).There was no significant correlation between Th17/Treg cell ratio and the patient's QMG score of disease severity(P>0.05).Conclusion:There is certain heterogeneity in Th17/Treg cell transformation and related cytokine levels in MG patients.
