摘  要:放射治疗在食管癌治疗中扮演了重要的角色。历经数十年,放疗技术取得了长足的进步,剂量适形性和摆位精准性不断提高,取得了较好的疗效。近年来,使用各种在线成像手段,在分次治疗前或治疗中获取患者影像,跟踪肿瘤靶区及周围器官位置、形状和密度的改变,引导调整摆位或修改治疗计划的影像引导放疗技术广泛进入临床应用。影像引导放疗可缩小治疗分次之间的摆位误差,有望进一步提高治疗增益比,减少放疗副反应。本文基于现有证据,对影像引导在食管癌放疗中的研究进展进行了综述,以期为该技术的临床推广应用提供参考。Radiotherapy plays an important role in the treatment of esophageal cancer.In the past few decades,great progress has been made in radiotherapy technology,and dose conformability and positioning accuracy have been continuously improved,achieving a better treatment response.In recent years,various online imaging methods have been used to obtain images of patients before or during fractionated treatment to track the changes in position,shape and density of tumor targets and organs at risk,and image-guided positioning adjustment or treatment plan modification has been widely used in clinical practice.Image-guided radiotherapy(IGRT)could reduce the setup errors of inter-fractional treatments,further improve the therapeutic gain factor,and reduce the side effects of radiotherapy.Based on the existing evidence,this article reviews the research progress of IGRT for esophageal cancer,in order to provide evidence for the clinical application of this technique.
