摘　　要：目的:观察患侧踝关节持续被动运动(CPM)同步健侧踝关节Brunnstrom联合反应技术在脑卒中后足下垂内翻治疗中的应用效果。方法:选取2021年5月—2022年7月广东祈福医院康复中心及神经内科收治的脑卒中患者84例,采用随机数字表法分为对照组与观察组各42例。对照组接受常规康复治疗和踝关节CPM机治疗。观察组在对照组基础上,患侧踝关节进行CPM治疗的同时同步健侧踝关节Brunnstrom联合反应技术,比较两组改良Ashworth痉挛评定量表(MAS)评分,Fugl-Meyer下肢运动功能评定量表(FMA-LE)评分,Berg平衡量表(BBS)评分,Holden步行功能分级(FAC)评分以及踝背屈活动度。结果:治疗8周后,观察组MAS、FMA-LE、BBS、FAC评分优于对照组,观察组患者踝背屈活动度高于对照组,观察组总有效率(92.85%)高于对照组(69.04%),差异均有统计学意义(P<0.05)。结论:患侧踝关节CPM同步健侧踝关节Brunnstrom联合反应技术治疗脑卒中后足下垂内翻,能有效提高脑卒中患者踝背屈能力,提高患者下肢整体运动功能,为改善脑卒中后患者足下垂的治疗提供有效方法。Objective To observe the application effect of the CPM of the affected ankle joint and the Brunnstrom combination of the healthy ankle joint in the treatment of foot droop varus after stroke.Methods A total of 84 stroke patients admitted to the Rehabilitation Center and Department of Neurology of Guangdong Clifford Hospital from May 2021 to July 2022 were selected and divided into 42 cases in the control group and observation group by random-number tables.The control group received conventional rehabilitation and ankle CPM machine treatment.In the observation group,the modified Ashworth Spasticity Rating Scale(MAS)score,Fugl-Meyer Lower Extremity Motor Function Rating Scale(FMA-LE)score,Berg Balance Scale(BBS)score,Holden walking function rating(FAC)score and ankle dorsiflexion range of motion were compared between the two groups.Results After 8 weeks of treatment,the MAS,FMA-LE,BBS and FAC scores in the observation group were better than those in the control group,and the ankle dorsiflexion range of patients in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The combination of CPM of the affected ankle joint and Brunnstrom of the healthy ankle joint in the treatment of foot droop and varus after stroke can effectively improve the ankle dorsiflexion ability of stroke patients and improve the overall motor function of the lower limbs,which is an effective method for improving the treatment of foot prolapse in patients after stroke.
