文拉法辛对帕金森病患者负性情绪及认知功能的影响

摘　　要：目的探讨合并焦虑抑郁的帕金森病患者应用文拉法辛治疗的疗效及对患者认知功能的影响。方法选取本院收治的合并焦虑抑郁的帕金森病患者119例,应用随机数字表法进行分组,对照组患者59例,给予常规药物联合心理干预进行治疗,观察组60例患者增加文拉法辛缓释胶囊辅助治疗,对比两组患者临床疗效、认知功能、症状、心理状态及不良反应。结果观察组患者总有效率高于对照组,事件相关电位P300潜伏期低于对照组,蒙特利尔认知评估量表(MoCA)评分、事件相关电位P300波幅、简易智力状态检查量表(MMSE)评分高于对照组,汉密顿抑郁量表(HAMD)评分、帕金森病综合评分量表(UPDRS)评分、汉密尔顿焦虑量表(HAMA)评分低于对照组(P<0.05);两组患者不良反应发生率无统计学意义(P>0.05)。结论文拉法辛对合并焦虑抑郁的帕金森病患者效果显著,有效缓解患者的焦虑抑郁状态,改善患者认知功能,安全性较好。Objective To analyze the effects of venlafaxine adjuvant therapy on mood and cognitive function in Parkinson’s disease patients combined with anxiety and depression. Methods A total of 119 Parkinson’s disease patients combined with anxiety and depression in our hospital were selected and divided by random number table. 59 patients in the control group were treated with conventional drugs and psychological intervention. 60 patients in the observation group were treated with venlafaxine. The clinical efficacy,cognitive function,symptoms,psychological status and adverse reactions of the two groups of patients were compared. Results The total effective rate of the observation group was higher than that of the control group,the event-related potential P300 latency was lower than that of the control group,the Montreal Cognitive Assessment Scale(MoCA)score,event-related potential P300 amplitude,and the Mini Mental State Examination Scale(MMSE)score were higher. In the control group,the scores of Hamilton Depression Scale(HAMD),Parkinson’s Disease Comprehensive Rating Scale(UPDRS)and Hamilton Anxiety Scale(HAMA)were lower than those of the control group(P<0.05). The incidence of adverse reactions of the two groups was not statistically different(P>0.05). Conclusion Venlafaxine has a significant effect in Parkinson’s disease patients combined with anxiety and depression. It can effectively relieve their anxiety and depression,improve their cognitive functio with highly safety.
