皮肤Rosai-Dorfman病15例临床及组织病理分析

摘　　要：目的:探讨皮肤Rosai-Dorfman病的临床和组织病理特征。方法:回顾性分析15例皮肤Rosai-Dorfman病患者的临床及组织病理资料。结果:15例患者中,男7例(46.7%),女8例(53.3%),发病年龄19~61岁,平均(40.2±12.5)岁,病程1个月~4年,平均(14.1±14.1)个月。皮损仅累及面部者4例,躯干者4例,四肢者3例,泛发全身者1例,同时累及面部和四肢者1例,累及躯干和四肢者2例。本组患者组织病理改变为真皮及皮下组织中淋巴细胞、组织细胞及浆细胞等呈弥漫性或灶状浸润,典型特征为“伸入运动”。免疫组化示组织细胞S-100蛋白阳性、CD68阳性及CD1a阴性。结论:皮肤Rosai-Dorfman病具有一定的临床及组织病理学特征,临床上应加强认识,减少误诊。Objective:To explore the clinical and pathological characteristics of cutaneous Rosai-Dorfman disease.Methods:The clinical and histopathological data of 15 cases of cutaneous Rosai-Dorfman disease were analyzed retrospectively.Results:Of the total 15 cases,seven cases were male(46.7%),and eight cases were females(53.3%).The average age of onset for the disease was 40.2±12.5 years(19-61 years).The average duration of disease was 14.1±14.1 months(1 month to 4 years).The disease affected almost any part of the body,including face(4 cases),trunk(4 cases),extremities(3 cases)and the whole body(1 case),the skin lesions affected both face and extremities in 1 case,and the trunk and extremities in 2 case.The histopathological features of 15 cases were similar.There was a patchy or diffusely cellular infiltrate composed of lymphocytes,histocytes and plasma cells,involving the dermis and sometimes the subcutis.A characteristic feature known as emperipolesis was noted.The immunohistochemical studies showed that the cellular infiltrate was positive for S-100 protein and CD68 but was negative for CD1a.Conclusion:Cutaneous Rosai-Dorfman disease has some clinical and histopathological characteristics that should be better recognized clinically to reduce misdiagnosis.
