摘 要:目的探讨阿利新蓝-过碘酸-雪夫(alcian blue/periodic acid-Schiff,AB-PAS)染色联合免疫组织化学检查在卵巢黏液性肿瘤中的应用研究。方法收集作者医院2018-08/2020-12月诊治的72例原发性卵巢黏液性肿瘤和12例转移性卵巢黏液性肿瘤患者为研究对象。免疫组织化学方法检测细胞角蛋白7(cytokeratin 7,CK7)、细胞角蛋白20(cytokeratin 20,CK20)、尾型同源核转录因子2(caudal-related homeobox transcription factor 2,CDX2)、富含AT序列特异性结合蛋白2(special AT-rich sequence-binding protein 2,SATB2)在肿瘤中的表达。AB-PAS染色检测肿瘤黏液的表达。结果原发性卵巢黏液性肿瘤患者年龄(42.7±15.1)岁小于转移性卵巢黏液性肿瘤患者年龄(51.1±8.6)岁,肿瘤最大直径(14.1±7.3)cm大于转移性卵巢黏液性肿瘤直径(10.3±5.2)cm(P均<0.05);原发性卵巢黏液性肿瘤发生部位[左侧39例(52.4%)、右侧33例(45.8%)]与转移性卵巢黏液性肿瘤[左侧1例(8.3%)、右侧5例(41.7%)、双侧6例(50.0%)]比较差异有统计学意义(P<0.05)。CK7在原发性卵巢黏液性肿瘤中的表达率(93.1%)显著高于转移性卵巢黏液性肿瘤(25.0%)(P<0.01)。CK20、CDX2、SATB2在原发性卵巢黏液性肿瘤中的表达率(27.8%、30.6%、1.4%)显著低于转移性卵巢黏液性肿瘤(100.0%、91.7%、91.7%)(P均<0.01)。AB-PAS染色在原发性卵巢黏液性肿瘤与转移性卵巢黏液性肿瘤表达差异具有统计学意义(P<0.01)。原发性卵巢黏液性肿瘤主要表达红色,转移性卵巢黏液性肿瘤显著表达蓝色。结论免疫组织化学联合AB-PAS染色的运用对于明确卵巢黏液性肿瘤的诊断具有支持和辅助意义。Objective To investigate the application of alcian blue/periodic acid-Schiff(AB-PAS)staining combined immunohistochemistry in ovarian mucinous tumors.Methods A total of 72 patients with primary ovarian mucinous tumors and 12 patients with metastatic ovarian mucinous tumors were collected from August 2018 to December 2020 in author′s hospital.The expression of cytokeratin 7(CK7),cytokeratin 20(CK20),caudal-related homeobox transcription factor 2(CDX2)and special AT-rich sequence-binding protein 2(SATB2)in tumors were detected by immunohistochemistry.The expression of tumor mucus was detected by AB-PAS staining.Results The age of patients with ovarian mucinous tumors(42.7±15.1)was lower than that of patients with metastatic ovarian mucinous tumors(51.1±8.6),and the tumor diameter of primary ovarian mucinous tumors(14.1±7.3)cm was larger than that of metastatic ovarian mucinous tumors(10.3±5.2)cm(all P<0.05);there were 39 cases(52.4%)of primary ovarian mucinous tumors on the left side and 33 cases(45.8%)on the right side,and 1 case(8.3%)of metastatic ovarian mucous tumors on the left,5 cases(41.7%)on the right and 6 cases(50.0%)on both sides,the difference between the two was statistically significant(P<0.05).The expression rate of CK7 in primary ovarian mucinous tumors(93.1%)was significantly higher than that in metastatic ovarian mucinous tumors(25.0%)(P<0.01).The expression rates of CK20,CDX2 and SATB2 in primary ovarian mucinous tumors(27.8%,30.6%,1.4%)were significantly lower than those in metastatic ovarian mucinous tumors(100.0%,91.7%,91.7%)(all P<0.01).The expression of AB-PAS staining was significantly different between primary ovarian mucinous tumors and metastatic ovarian mucinous tumors(P<0.01).Primary ovarian mucinous tumors were mainly expressed in red,while metastatic ovarian mucinous tumors were significantly expressed in blue.Conclusion Immunohistochemistry combined AB-PAS staining is of more support and auxiliary significance in the diagnosis of ovarian mucinous tumor.
