摘　　要：目的系统评价影响肠造口术后患者生命质量的主要因素。方法检索中国知网、维普、万方、PubMed、Embase数据库中关于造口术后患者生命质量影响因素研究的文献,各数据库检索日期从建库到2022年3月10日。采用RevMan 5.4软件对符合纳入条件的文献进行分析。结果共纳入15篇文献,包含3769例患者。肠造口患者生命质量的影响因素合并效应量及95%CI分别为费用支付方式[SMD=-0.21;95%CI(-0.39,-0.02);P=0.03]、是否渗漏[SMD=-0.34;95%CI(-0.44,-0.23);P<0.01]、排便规律[SMD=0.38;95%CI(0.24,0.51);P<0.01]、造口并发症[SMD=-0.69;95%CI(-1.01,-0.36);P<0.01]。结论费用支付方式、是否渗漏、排便规律及造口并发症是影响肠造口患者生命质量的主要因素。受纳入研究数量及质量的限制,未来需更多研究进一步加以验证。Objective To systematically evaluate the main factors affecting the quality of life of patients after enterostomy.Methods The literature on the influencing factors of life quality in patients after ostomy was retrieved from CNKI,Weipu,Wanfang,PubMed and Embase databases,and the search dates of each database were from the establishment of the database to March 10,2022.RevMan 5.4 software was used to analyze the literature eligible for inclusion.Results A total of 15 articles with a sample size of 3769 cases were included in strict accordance with the inclusion and exclusion criteria.The amount of effects and 95%CI of the influencing factors of life quality in patients with enterostomy were the method of payment of expenses[SMD=-0.21;95%CI(-0.39,-0.02);P=0.03],whether there was leakage[SMD=-0.34;95%CI(-0.44,-0.23);P<0.01],defecation pattern[SMD=0.38;95%CI(0.24,0.51);P<0.01],and ostomy complications[SMD=-0.69;95%CI(-1.01,-0.36);P<0.01].Conclusion The method of payment,whether there is leakage,regularity of bowel movement,and ostomy complications are the main factors affecting the quality of life of enterostomy patients.Limited by the number and quality of included studies,more studies are needed for further validation in the future.
