摘 要:目的 调查中国六个地区成熟母乳中叶酸的含量以及形式分布。方法 本研究是母爱研究的一部分。在兰州、天津、上海、广州、成都以及长春六个城市招募单胎足月分娩的健康产妇共319例,收集40~45d成熟乳,采用液相色谱-串联质谱法(LC-MS)定量分析母乳中5-甲基四氢叶酸、未代谢合成叶酸(unmetabolized folic acid,UMFA)、四氢叶酸、5,10-亚甲基四氢叶酸以及5-甲酰四氢叶酸含量。由于四氢叶酸,5,10-亚甲基四氢叶酸以及5-甲酰四氢叶酸存在相互转化的可能,将这三种叶酸合并称为其他还原型叶酸。这五种叶酸形式合并称为总叶酸。结果 在319例成熟乳样本中5-甲基四氢叶酸、其他还原型叶酸中位数含量分别为31.63、15.62ng/ml。UMFA的中位数含量小于检出限(0.40ng/ml),其中137(42.94%)份母乳被准确定量(定量限=1.34ng/ml)。母乳中总叶酸中位数含量为54.06 ng/ml。我国成熟乳中5-甲基四氢叶酸含量在总叶酸中占比最高(59.70%),其他还原型叶酸次之(32.18%),UMFA最少(8.12%)。六地区成熟乳中5-甲基四氢叶酸、其他还原型叶酸、UMFA以及总叶酸含量均存在统计学差异(P<0.05),其中广州和成都地区成熟乳中5-甲基四氢叶酸含量高于其他地区(P<0.001)。结论 在中国六个城市成熟乳中,总叶酸含量存在地区间差异,其中5-甲基四氢叶酸的水平在总叶酸中占绝对优势。[营养学报,2022,44(5):430-435]Objective The study aimed to profile the composition of folates in mature breast milks from six regions of China and assess the concentrations of individual components and total folate. Methods This study is part of the Maternal Nutrition and Infant Investigation(MUAI) Study. A total of 319 samples of mature breast milk samples(40-45 days postpartum) were collected from healthy lactating mothers of singleton full-term infants from six regions in China. The contents of tetrahydrofolate(THF), 5-methyl-THF, 5,10-methenyl-THF, 5-formyl-THF and unmetabolized folic acid(UMFA)in breast milk were determined by using liquid chromatography-tandem mass spectrometry(LC-MS). Due to the possibility of mutual transformation among 5,10-methenyl-THF, THF, and 5-formyl-THF, the contents of these three species of folate were combined in this study and called the other reduced folates. The sum of these folates was reported as total folate.Results Of all tested 319 mature milk samples, the median values of 5-methyl-THF, other reduced folates were 31.63 ng/ml,15.62 ng/ml, respectively. UMFA was only detected in 42.94% of breast milk samples with an LoD of 1.34ng/ml, so the median level of UMFA was as below LoD. The median value of total folate was 54.06 ng/ml. 5-methyl-THF was the dominant form in total folate(59.70%) followed by other reduced folates(32.18%) and UMFA(8.12%). The contents of 5-Methyl-THF, UMFA, other reduced folates and total folate were significantly different among different regions(P<0.001).Conclusion 5-methyl-THF was the dominant form of folate in Chinese mature breast milk.
