摘　　要：目的分析轻链型多发性骨髓瘤(light chain multiple myeloma,LCMM)中血、尿轻链与M蛋白的相关性,探讨其在LCMM诊断中的应用价值。方法收集104例初诊LCMM患者的临床资料及实验室检测结果,分析血、尿轻链与M蛋白的相关性。结果LCMM患者分为有M蛋白组与无M蛋白组,两组患者血、尿受累轻链及κ/λ比值的差异具有统计学意义(P均<0.05);M蛋白水平与血受累轻链含量的相关性显著(P均<0.001)。结论血、尿受累轻链及κ/λ比值是影响LCMM患者血清中是否出现M蛋白条带的重要影响因素,为LCMM患者的临床诊断提供了有力的实验室依据。Objective To analyze the correlation between blood and urine light chain and M protein in light chain multiple myeloma(LCMM),and to explore its application value in the diagnosis of LCMM.Methods The clinical data and laboratory test results of 104 newly diagnosed LCMM patients were collected,and the correlation between hematuria light chain and M protein was analyzed.Results LCMM patients were divided into groups with and without M protein.Compared with two groups,there was significant difference in hematuria affecting light chain andκ/λratio(all P<0.05);the correlation between the proportion of M protein and the content of serum involved light chains was strong(all P<0.001).Conclusion Blood and urine light chain involvement andκ/λratio are important influencing factors for the presence of M protein band in serum of patients with LCMM,which provides a strong laboratory basis for the clinical diagnosis of patients with LCMM.
