健脾补精汤联合个体化干预改善胃癌术后脾虚证患者免疫功能的研究

摘　　要：目的探讨健脾补精汤联合个体化干预在胃癌术后脾虚证患者中的应用效果。方法选取2021年4月—2022年3月在南阳市第一人民医院就诊的152例胃癌术后脾虚证患者为研究对象。以随机数字表法为分组依据,每组76例。对照组给予早期胃肠营养支持与个体化干预,试验组在此基础上联合健脾补精汤。结果与对照组比较,试验组排气、排便、置管时间更短,总引流量更低。与治疗前比较,治疗后2组中医各项证候积分降低,且试验组更低;试验组外周血CD4^(+)水平升高,对照组降低,则试验组更高;试验组外周血CD8^(+)水平降低,对照组升高,则试验组更低;2组血清胃动素、胃泌素、血管活性肠肽水平升高,且试验组更高(P<0.05)。结论健脾补精汤联合个体化干预可有效帮助缩短胃癌术后脾虚证患者恢复时间,缓解其临床症状,还可改善其胃肠功能以及免疫功能。Objective To explore the application effect of Jianpi Bujing decoction combined with individualized intervention in patients with spleen deficiency syndrome after gastric cancer operation.Methods From April 2021 to March 2022,152 patients with spleen deficiency syndrome after gastric cancer surgery in the First People’s Hospital of Nanyang were selected as the research objects.There were 76 cases in each group according to the random number table method.The control group was given early gastrointestinal nutrition support and individualized intervention,and the experimental group was combined with Jianpi Bujing decoction on this basis.Results Compared with the control group,the exhaust,defecation and catheterization time of the experimental group were shorter,and the total drainage volume was lower.Compared with before treatment,the scores of TCM syndromes in the two groups decreased after treatment,and the scores in the experimental group were lower.The level of CD4^(+)in peripheral blood of the experimental group was increased,while that of the control group was decreased.The level of CD8^(+)in peripheral blood of the experimental group was decreased,while that of the control group was increased.The serum levels of motilin,gastrin and vasoactive intestinal peptide in the two groups were increased,and the experimental group was higher(P<0.05).Conclusion Jianpi Bujing decoction combined with individualized intervention can effectively help shorten the recovery time of patients with spleen deficiency syndrome after gastric cancer surgery,relieve their clinical symptoms,and improve their gastrointestinal function and immune function.
