培元益气汤改善气阴两虚证胃癌相关癌性疲乏及对患者免疫功能的影响

摘　　要：目的:探讨培元益气汤改善气阴两虚证胃癌的相关癌性疲乏及对患者免疫功能的影响。方法:选取112例气阴两虚证胃癌患者,随机分为观察组和对照组各56例,对照组治疗方案为替吉奥加顺铂化疗,观察组在对照组基础上加以培元益气汤治疗,观察两组患者癌性疲乏及免疫功能指标的变化。结果:治疗21d后,观察组临床疗效为96.43%,对照组为78.57%,两组比较差异有统计学意义(P<0.05)。对照组治疗后简易疲劳量表(BFI)评分低于治疗前,观察组BFI评分低于治疗前(P<0.05);治疗后观察组BFI评分低于对照组(P<0.05)。治疗后观察组CD3^(+)、CD4^(+)表达高于对照组,CD8^(+)表达低于对照组,皮质醇(Cor)水平高于对照组,差异有统计学意义(P<0.05)。治疗后观察组体力状况评分较对照组更高(P<0.05);治疗后两组患者PIPER-疲乏量表评分低于治疗前,差异具有统计学意义(P<0.05);其中观察组PIPER-疲乏量表评分低于对照组(P<0.05)。两组患者治疗后生活质量评分优于治疗前(P<0.05),观察组改善优于对照组(P<0.05)。结论:经培元益气汤治疗后可显著改善气阴两虚证胃癌疲乏疗效,提高患者免疫功能及血清皮质醇水平。Objective:To explore and study the effect of Peiyuan Yiqi decoction on improving cancer-related fatigue and immune function of gastric cancer with qi-yin deficiency syndrome.Methods:112 patients with gastric cancer with deficiency of both qi and yin were divided into observation group(56 cases)and control group(56 cases)randomly.The control group was treated with Tegiol plus cisplatin chemotherapy while the observation group was treated with Peiyuan Yiqi decoction.The effects of cancer fatigue and immune function were observed.Results:After 21 days of treatment,the effective rate was 96.43%in observation group was higher than control group(78.57%,P<0.05).After treatment,the Brief fatigue inventory(BFI)score was decreased in control group.The expression of CD3^(+)and CD4^(+)were higher than control group.Observation group displayed lower level of CD8^(+).Cortisol level in observation group was higher than control group(P<0.05).Observation group showed better physical condition compared with control group(P<0.05).After treatment,the score of PIPER-fatigue scale in control group was lower than that in observation group(P<0.05).The PIPER-fatigue scale score of observation group was lower than that of control group(P<0.05).After treatment,the quality of life scores of the two groups were significantly improved compared with baseline condition(P<0.05).In addition,subjects in observation group had better score(P<0.05).Conclusion:After treatment with Peiyuan Yiqi decoction,qi can be significantly improved the further clinical application in the treatment of gastric cancer with qi and yin deficiency syndrome and fatigue.
