年龄相关性白内障合并浅前房患者术后综合性视觉功能指数及影响因素分析

摘　　要：目的:探讨影响年龄相关性白内障合并浅前房患者手术后综合性视觉功能指数的因素。方法:选择进行手术治疗的年龄相关性白内障合并浅前房患者80例为研究对象。术后使用综合性视觉功能指数(VF-7)问卷法评估所有患者视觉功能,并依此将患者分为VF-7评分≤90分组和VF-7评分>90分组,通过单因素和多因素Logistic回归分析探讨影响年龄相关白内障合并浅前房患者手术后综合性视觉指数的因素。结果:80例患者术后VF-7评分低于75分、75~90分和高于90分的患者分别为23例(28.75%)、30例(37.50%)和27例(33.75%)。不同VF-7评分患者术前在年龄、性别、BMI、眼轴长度、角膜曲率、眼压和晶体混浊度值等比较差异无统计学意义(均P>0.05)。单因素与多因素Logistic回归分析显示:合并老年黄斑病变(OR=2.261,95%CI:1.028~3.619)、高度近视性视网膜病变(OR=2.261,95%CI:1.028~3.619)、糖尿病性视网膜病变(OR=2.261,95%CI:1.028~3.619)以及青光眼性视神经萎缩(OR=2.261,95%CI:1.028~3.619)是影响年龄相关性白内障合并浅前房术后综合性视觉指数的独立危险因素。结论:年龄相关性白内障合并浅前房患者手术治疗后综合性视觉功能指数受高度近视性视网膜病变、黄斑病变以及糖尿病性视网膜病变等多种眼部疾病影响。Objective:To study the factors affecting the comprehensive visual-functioning index after surgery in patients with age-related cataract and shallow anterior chamber.Methods:A total of 80 patients with age-related cataract and shallow anterior chamber who underwent surgical were selected as the research subjects.The visual function of all patients was evaluated by the comprehensive visual-functioning index(VF-7)questionnaire after surgery,and was divided into VF-7≤90 group and VF-7>90 group.Univariate and multivariate Logistic regression analysis were used to explore the factors affecting the comprehensive visual-functioning index after surgery in patients with age-related cataract and shallow anterior chamber.Results:Among the 80 age-related cataract patients with shallow anterior chamber,23(28.75%),30(37.50%)and 27(33.75%)had VF-7 scores lower than 75,75 to 90 and higher than 90,respectively.There was no significant difference in age,gender,BMI,axial length,corneal curvature,intraocular pressure and lens opacity among patients with different VF-7 scores before surgery(all P>0.05).Univariate and multivariate Logistic regression analysis showed that age-related macular degeneration(OR=2.261,95%CI:1.028 to 3.619),high myopic retinopathy(OR=2.261,95%CI:1.028 to 3.619),diabetic retinopathy lesions(OR=2.261,95%CI:1.028 to 3.619)and glaucomatous optic atrophy(OR=2.261,95%CI:1.028 to 3.619)were independent risks affecting comprehensive visual-functioning index after age-related cataract with shallow anterior chamber.Conclusion:The comprehensive visual-functioning index of patients with age-related cataract and shallow anterior chamber after surgical treatment is affected by various eye diseases such as high myopia retinopathy,macular degeneration and diabetic retinopathy.
