补气化瘀汤联合针灸方案对脊髓损伤患者术后神经功能的康复疗效及BDNF、IGF-1表达的影响

摘　　要：目的 探讨补气化瘀汤联合针灸方案对脊髓损伤患者术后神经功能的康复疗效及BDNF、IGF-1表达的影响。方法 选取2018年2月—2020年2月就诊于惠州市第一、第三及中心医院康复科行脊髓损伤修复术后患者共110例作为研究对象,随机分为观察组和对照组各,55例。给予基础康复治疗的基础上,观察组给予针灸联合补气化瘀汤治疗;对照组单用针灸治疗。观察两组治疗前后的ASIA评分、Barthel指数评分、IGF-1与BDNF表达水平。结果 治疗前,两组的ASIA评分、Barthel指数评分、IGF-1和BDNF表达水平差异无统计学意义(P>0.05);治疗后,观察组ASIA评分从(88.32±5.96)分提高到(162.17±11.33)分、Barthel指数评分从(55.27±4.82)分提高到(89.14±7.72)分、IGF-1指标水平从(77.26±9.67)μg/mL升至(108.37±7.82)μg/mL、BDNF指标水平从(3.84±0.47)ng/mL升至(4.99±0.59)ng/mL,升高水平明显优于对照组,差异有统计学意义(P<0.05)。结论 采用补气化瘀汤联合针灸方案治疗脊髓损伤患者,可明显改善其术后的神经功能,同时增强BDNF、IGF-1表达,有良好疗效。Objective To explore the effect of combining Qi-supplementing and stasis-transforming soup with acupuncture protocol on the rehabilitation efficacy of postoperative neurological function and the expression of BDNF,IGF-1 in patients with spinal cord injury. Methods A total of 110 patients who attended the rehabilitation departments of the first, third and central hospitals in Huizhou City after spinal cord injury repair from February 2018 to February 2020 were selected as study subjects. The cases were randomly divided into observation group and control group, with 55 cases in each group. Based on the basic rehabilitation treatment, the observation group was treated with acupuncture combined with qi-supplementing and stasis-removing soup. The control group was treated with acupuncture alone, and the ASIA score, Barthel index score, IGF-1 and BDNF expression levels of the two groups were observed before and after treatment. Results Before treatment, there were no statistical differences in ASIA score, Barthel index score, IGF-1 and BDNF expression levels between the two groups(P>0.05);after treatment, ASIA score in the observation group improved from(88.32±5.96) points to(162.17±11.33) points, Barthel index score improved from(55.27±4.82) points to(89.14±7.72) points, IGF-1 index level increased from(77.26±9.67) μg/mL to(108.37±7.82)μg/mL, BDNF index levels increased from(3.84±0.47) ng/mL to(4.99±0.59) ng/mL, and the elevated levels were significantly better than those in the control group, the difference was statistically significant(P<0.05). Conclusion Treatment of spinal cord injury patients with a combination of tonic qi and stasis soup and acupuncture protocol can significantly improve their postoperative neurological function, and can also enhance BDNF and IGF-1 expression with good efficacy.
