摘  要:新生儿坏死性小肠结肠炎(NEC)是新生儿期严重的肠道炎症性疾病,死亡率较高,发病率与胎龄呈负相关,且存活患儿后期遗留严重的后遗症,导致生活质量下降及长期心理问题,但因其早期表现不典型且发病机制尚不明确,导致NEC在早期预防及诊断中存在一定困难。越来越多的研究表明肠道菌群失调、肠道黏膜屏障破坏、先天性免疫调节异常是NEC发病的基础,而双歧杆菌是新生儿肠道共生菌中的优势菌群,具有抗病原、抗炎、免疫调节等生物学功能,在预防及减轻NEC中作用显著,但其在预防NEC中的潜在机制仍在深入研究中,现将双歧杆菌目前在防止NEC中的作用机制做一综述,为NEC的预防及诊治提供理论依据。Neonatal necrotizing enterocolitis(NEC)is a severe intestinal inflammatory disease in the neonatal period.The mortality rate is high,and the incidence is negatively correlated with gestational age.In addition,the surviving children have serious sequelae in the later stage,leading to the deterioration of quality of life and long-term psychological problems.However,due to its atypical early manifestations and unclear pathogenesis,there are certain difficulties in the early prevention and diagnosis of NEC.More and more studies have shown that dysregulation of intestinal flora,destruction of intestinal mucosal barrier,and abnormal innate immune regulation are the basis of NEC pathogenesis.Bifidobacterium is the dominant bacterial group in neonatal intestinal symbiosis,which has biological functions such as antiviral agents,anti-inflammatory,and immune regulation and plays a significant role in the prevention and alleviation of NEC.However,its potential mechanism in the prevention of NEC is still under in-depth study.This article reviews the mechanism of Bifidobacterium in preventing NEC,so as to provide theoretical basis for the prevention,diagnosis and treatment of NEC.
