超声剪切波弹性成像对甲状腺癌包膜侵犯、淋巴结转移的诊断价值及其与促甲状腺素水平的相关性

摘　　要：目的研究超声剪切波弹性成像对甲状腺癌包膜侵犯、淋巴结转移的诊断价值及其与促甲状腺素水平的相关性。方法选取142例甲状腺癌患者作为实验组(n=142)以及同期141例甲状腺良性结节患者作为对照组(n=141)。实验组患者根据检查结果包膜是否累及中央区分为包膜侵犯组和无包膜侵犯组,根据患者是否存在淋巴结转移分为淋巴结转移组和无淋巴结转移组,所有患者均接受超声剪切波弹性成像(SWE)检查。对比包膜侵犯组和无包膜侵犯组患者SWE参数Emax、Emin和Emean;对比淋巴结转移组和无淋巴结转移组患者SWE参数Emax、Emin和Emean;对比实验组和对照组患者促甲状腺素水平(TSH),采用spearson分析超声剪切波弹性成像参数与TSH水平的相关性。结果与无包膜侵犯组相比,包膜侵犯组患者SWE参数Emax、Emin和Emean显著更高(P<0.05);与无淋巴结转移组患者相比,淋巴结转移组患者SWE参数Emax、Emin和Emean显著更高(P<0.05)。与对照组患者相比,实验组患者TSH水平显著更高(P<0.05);采用spearson分析得出:SWE参数Emax、Emin和Emean与患者TSH水平呈正相关(P<0.05)。结论超声剪切波弹性成像对甲状腺癌包膜侵犯、淋巴结转移具有重要的诊断价值,其参数与患者TSH呈正相关。Objective To evaluate the diagnostic value of shear wave elastography(SWE)for capsular invasion and lymph node metastasis in patients with thyroid cancer,and to analyze its correlation with thyroid stimulating hormone(TSH)level.Methods A total of 142 patients with thyroid cancer were enrolled as the experimental group,meantime,another 141 patients with thyroid benign nodules were set as the control group.Patients in the experimental group were further divided into subgroups according to the presence or absence of capsular invasion and central lymph node metastasis,invasion subgroup,non-invasion subgroup,metastasis subgroup,and non-metastasis subgroup.All selected individuals received SWE examinations,and the parameters including maximum,mean,and minimum elastic modulus(Emax,Emean,Emin)of 4 subgroups were compared.Serum levels of TSH was compared between the experimental group and the control group.Then Spearson’s correlation analysis was used to verify the relationship between the SWE parameters and TSH level in patients with thyroid cancer.Results Compared with the non-invasion subgroup,SWE parameters Emax,Emin and Emean were significantly higher in invasion group(P<0.05).Compared with non-metastasis subgroup,SWE parameters Emax,Emin and Emean of metastasis group were significantly higher(P<0.05).TSH level of the experimental group was significantly higher than that of the control group(P<0.05).Correlation analysis showed that SWE parameters Emax,Emin and Emean were positively correlated with TSH level(P<0.05).Conclusion SWE has great diagnostic value for capsular invasion and lymph node metastasis in patients with thyroid cancer,moreover,its parameters have positive correlation with TSH levels in patients.
