大面积膀胱肿瘤患者TURBT术后膀胱复发的相关危险因素分析

摘　　要：目的分析大面积膀胱肿瘤患者TURBT术后膀胱复发的相关危险因素。方法回顾性分析128例行TURBT治疗的大面积膀胱癌患者的一般资料,包括性别、年龄、吸烟史、肿瘤数目、肿瘤部位、是否维持膀胱灌注、肿瘤分级等一般资料,连续随访36个月,记录患者复发情况,并采用单因素和多因素Logistic回归分析术后复发的相关危险因素。结果2组性别、年龄、吸烟史、体重指数、肿瘤部位、是否维持膀胱灌注比较,差异无统计学意义(P>0.05);2组肿瘤数目、肿瘤分期、肿瘤分级、肿瘤形态比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,肿瘤数目>2个、肿瘤分期高、肿瘤级别高、肿瘤形态为宽基均是TURBT术后复发的独立危险因素。结论肿瘤数目>2个、肿瘤分期高、肿瘤级别高、肿瘤形态为宽基均是TURBT术后复发的独立危险因素,故对于具备以上特征的患者应给予高度关注,采取更加积极的治疗策略,最大程度减少复发。Objective To analyze the risk factors of TURBT bladder recurrence in patients with large bladder tumors.Methods Retrospective analysis of 128 patients with large bladder cancer who underwent TURBT treatment were conducted,including gender,age,smoking history,tumor number,tumor site,whether to maintain bladder perfusion,tumor classification,followed up for 36 months,recorded recurrence,and analyzed risk factors after Logistic regression.Results Gender,age,smoking,history,body mass index,tumor site,bladder perfusion of the 2 groups had no significant difference(P>0.05),tumor number,tumor staging and tumor classification of the 2 groups had significant difference(P<0.05).Multi-factor Logistic regression analysis showed that the number of tumors>2,high tumor stage,high tumor grade and wide tumor morphology were all independent risk factors for TURBT recurrence.Conclusion The tumor number>2,high tumor stage,high tumor grade and wide tumor form are independent risk factors for TURBT recurrence.Therefore,the patients with the above characteristics should be paid high attention,provide better personalized treatment plan,and strengthen postoperative follow-up to maximize the survival time.
