儿童腹膜后畸胎瘤围手术期并发症的相关危险因素分析

摘　　要：目的分析儿童腹膜后畸胎瘤围手术期并发症的相关危险因素。方法选取56例儿童腹膜后畸胎瘤围手术期发生并发症的患儿作为发生组,并以1∶1配比选取56例同期儿童腹膜后畸胎瘤围手术期未发生并发症的患儿作为未发生组。收集两组性别、年龄、体重、肿瘤位置、跨越中线、手术时长、直径、病理、COG分期、静脉移位、器官移位等信息,分析儿童腹膜后畸胎瘤围手术期发生并发症的危险因素。结果年龄、体重、跨越中线、直径、静脉移位、器官移位是影响儿童腹膜后畸胎瘤围手术期发生并发症的单因素(P<0.05);logistic回归分析显示,年龄≤1岁、体重>5 kg、跨越中线、直径长、静脉移位、器官移位是儿童腹膜后畸胎瘤围手术期发生并发症的影响因素(P<0.05)。结论儿童腹膜后畸胎瘤围手术期发生并发症的影响因素较多,临床应根据上述因素实施相关措施,做好术前影像学检查及评估,提高手术安全性,减少并发症发生。Objective To analyze the relevant risk factors for perioperative complications of pediatric retroperitoneal teratoma.Methods 56 children with perioperative complications of retroperitoneal teratoma were selected as the occurrence group,and 56 children with retroperitoneal teratoma without perioperative complications were selected as the non occurrence group.Gender,age,body weight,tumor location,the midline crossing,operation length,diameter,pathology,COG stage,vein displacement,and organ displacement was collected from both groups to analyze the risk factors for perioperative complications in pediatric retroperitoneal teratoma.Results Age,body weight,crossing midline,diameter,vein displacement,and organ displacement were single factors(P<0.05)affecting the perioperative complications of pediatric retroperitoneal teratoma;logistic regression analysis showed that age of 1 year,body weight>5 kg,crossing midline,long diameter,vein displacement,and organ displacement were the influencing factors(P<0.05).Conclusion There are many factors affecting the perioperative complications of retroperitoneal teratoma in children.Clinically,relevant measures should be implemented according to the above factors,and the preoperative imaging examination and evaluation should be done to improve the surgical safety and reduce the occurrence of complications.
